JOINT AFFIDAVIT
OF TWO DISINTERESTED PERSONS
WE, REMEDIOS D. DACAYANAN and RUBENITA D.
QUIAMBAO, of legal ages, Filipino citizens and residents of
Barangay Sta. Maria, Camiling, Tarlac, Philippines, after having been
duly sworn to in accordance with law, do hereby depose and state
THAT:

1. We both personally know HILDA ABRAZADO DELA CRUZ

(Hilda, for brevity), who was born on December 30, 1958 to
Spouses Apolonio Dela Cruz and Felicidad Abrazado in
Camiling, Tarlac;

2. Hilda is also known as MRS. HILDA DELA CRUZ DOMINGO

after she got married to Nestor J. Domingo (Nestor);

3. Nestor is a member of the Social Security System of the

Philippines (SSS) until his death on April 21, 2017;

4. When Hilda went to the SSS Camiling Branch to transfer the

pension benefit of Nestor in her favor, she was required to
submit copies of her Certificate of Live Birth, Marriage
Contract, CRS Form No.5 Advisory on Marriages, and the
certificates of birth and death of Nestor;

5. Upon obtaining the required documents, it was discovered in

the said Marriage Contract that the recoded Name of Hildas
mother is FELICITASCION ABRAZADO, instead of
FELICIDAD ABRAZADO or FELICIDAD MARIANO
ABRAZADO as appearing in her Certificate of Live Birth and
CRS Form No. 5 Advisory on Marriages;

6. Meanwhile, the recorded Name of Nestors mother appearing

in said Marriage Contract and Certificate of Death is
VICTORIANA JOSE, instead of VICTORINA TUMBAGA
JOSE as appearing in his Certificate of Live Birth;

7. We are not related to Hilda and Nestor by affinity or

consanguinity, except that of being their friends;
 8. We are executing this Affidavit to attest to the fact that the
names FELICIDAD ABRAZADO, appearing as the mother of
Hilda Dela Cruz in her Certificate of Live Birth,
FELICITASCION ABRAZADO, appearing as the mother of
Hilda Dela Cruz in the said Marriage Contract, and FELICIDAD
MARIANO ABRAZADO, appearing as the mother of Hilda
Abrazado Dela Cruz in the said CRS Form No. 5 Advisory on
Marriages, refer to one and the same person, and that is the
biological mother of our friend Hilda;

9. Moreover, we are executing this Affidavit to attest to the fact

that the names VICTORINA TUMBAGA JOSE, appearing as
the mother of Nestor Jose Domingo in Nestors Certificate of
Live Birth, and VICTORIANA JOSE, appearing as the mother
of Nestor Jose Domingo in the above-stated Marriage Contract
and Certificate of Death, refer to one and the same person,
and that is the biological mother of our friend Nestor;

10. Finally, we are executing this Affidavit to attest to the truth

of the foregoing statements and for all legal intents and
purposes it may serve.

IN WITNESS WHEREOF, we have hereunto affixed our

signatures this 20th day of June, 2017 at Camiling, Tarlac,
