AFFIDAVIT

I, BELEN T. ELIZAGA, Filipino, of legal age, married, and a resident of Leo St.,
Villa Ernesto subd., Gusa, Cagayan de Oro City, Philippines, after being sworn in
accordance with law, hereby depose and say that:

1. I am the registered subscriber of the Misortel telephone no. 721283

located in Mandumol, Cagayan de Oro City;

2. When I applied for the telephone line in October 9, 1998, I paid a deposit
of FIVE THOUSAND PESOS (P5,000.00) to Misortel. For the said
deposit, Misortel issued an Official Receipt (O.R.) with O.R. no. 67365;

3. The said O.R. was misplaced by me and after exerting earnest efforts to
recover the same, I still could not find it;

4. I now consider the said O.R. as lost for all intents and purposes;

5. I execute this affidavit to attest to the truth of the foregoing facts in order to
claim my telephone deposit of P5,000.00 and for any other legal intents
and purpose which it may serve.
