Examples of Questions and Scales for Post-activity Questionnaires


This template provides you with examples of different types of questions and scales that can be
considered in designing a post-activity questionnaire.

It is recommended that you keep to using one type of scale so as not to confuse the respondents.

You can cut and paste the question formatting into a new document and modify as needed.

5-point scale
This example uses a five-point scale from strongly agree to strongly disagree.

QUESTIONS ABOUT THE PRESENTER

Please state the extent to which you agree or disagree with the following statements, where 1 is
Strongly Agree and 5 is Strongly Disagree (tick one per statement).

4-point Scale
This example uses a four-point scale from excellent to poor (alternatively, it could range from
excellent to needs improvement). Even number scales such as this do not allow the respondent to
select a mid-point.

7-point Scale
This example uses 7-point scale from very satisfied to not at all satisfied.

Please select a rating that best reflects your opinion on the following aspects of the workshop,
where 1 is Very Satisfied and 7 is Not at all satisfied (tick one per statement).

This example uses 7-point scale that presents the respondents with range of answers to complete a
statement.

2-option Response
This example allows respondents to answer from two choices. The choices can be Yes-No, Agree-
Disagree, or True-False.
