having been sworn in accordance with law do hereby deposes and state:

1. That I am the vendor of a certain motor vehicle which is more particularly describe as follows:

2. That the vendee of the above-described motor vehicle was the Motor Ace Philippines
Incorporated (MAPI) and the said Deed of Absolute Sale was made and executed on
September 5, 2016 in the City of Puerto Princesa, duly notarized by Atty. Philip Ray L. Nangkil;

3. That the Deed of Absolute Sale that I have signed on signed September 5, 2016 and the
Release of Chattel Mortgage that I have signed on June 2, 2016, my signature my signature in
the two documents that I have signed was different;

4. That I hereby guarantee, that I DONNA Q. GALONO, who signed the Deed of Absolute Sale
dated September 2, 2016 and DONNA Q. GALONO, who signed the Release of Chattel
Mortgage on June 2, 2016, is one and the same person and that I have caused to signed the
above-mentioned documents on the respective dates indicated on the said documents;

5. That I executed this affidavit to attest to the truthfulness of the foregoing facts and for
whatever legal purpose the same may serve.
