SAFETY ALERT

INCIDENT DESCRIPTION:
The injured person(IP) was walking on the
middle of the site access road when he was hit
by a reversing pick-up truck. IP was
immediately transported by ambulance to the
hospital for treatment. Radiological test results
revealed that he sustained a fracture to his
right knee cap, subsequently, affected leg has
been placed on a cast.
Prior to the incident, the pick-up truck driver
was manoeuvring the vehicle in reverse
direction on a rut soil. As it got over the soft
ground and the tires regained traction, the
truck jumped causing the driver to
momentarily lost control of the vehicles
acceleration. In addition, the driver was not
able to see the approaching pedestrian because
his line of sight was obstructed by an
earthmoving equipment(JCB) working at the
rear end of his pick up truck.

ACTIONS TO PREVENT RECURRENCE OF


SIMILAR INCIDENT IN THE PNG LNG
EPC3 PROJECT:
Introduce the use of pedestrian walk ways
and side walks. Do Not walk in the middle
of the road.
Reversing with out a banksman should be
prohibited on the construction site.
If reversing is required, ensure that a
banksman is present and controlling the
area (on-coming foot /vehicle traffic).
FACTORS THAT CAUSED THE INCIDENT: Only drive in reverse as your last option,
No banksman was present to assist use turn around points as much as
the pick-up truck driver. possible.
There was no sidewalk or pedestrian
lane.
