    Dear Dr. Limson:

    I would like to request a five (5) days leave of absence for personal reasons.

    If possible, I would like to leave work on May 29, 2017 and return on June 5, 2017.

    If approved, I will be staying with my family in Samar during this time period and I would be
    glad to assist with any questions via phone whenever possible.
