Risk matrix use this to determine

risk for each hazard i.e. how bad and how Likelihood of Harm
likely
Remote Very unlikely Unlikely Possible Likely
Severity of Harm e.g. <1 in 1000 e.g. 1 in 200 e.g. 1 in 50 e.g. 1 in 10 e.g. >1 in 3
chance chance chance chance chance
Negligible e.g. small bruise Trivial Trivial Trivial Low Low
Slight e.g. small cut, deep bruise Trivial Trivial Low Low Medium
Moderate e.g. deep cut, torn muscle Trivial Low Medium Medium High
Severe e.g. fracture, loss of Low Medium High High Extremely high
consciousness
Very Severe e.g. death, permanent Low Medium High Extremely Extremely high
disability high
Risk Assessment Form Part B: Hazards & Control
Measures
Hazards Who exposed Risk Control measures Risk

Equipment Falling over Cast and Crew High Equipment will be used by those who are most confident in using the equipment low
and a clearway will be made so there is only one person handling the equipment
at a time to prevent knocking the equipment
Trip Hazard Cast and Crew High Wires may be used for lighting causing a trip hazard to members of the public Medium
and cast and crew. To reduce this risk well section the cables off or make it clear
that theres a cable there by getting someone to stand near to ask the public to
walk around the potential trip hazard
Crew High Some of the equipment is very heavy so we need to designate the stronger Medium
Manual Handling members of the crew to carry the heavier equipment to prevent the weaker ones
injuring themselves. Or we need to get two people to carry one piece of
equipment to share the weight.
Slippery Surface Crew and Cast Medium A slippery surface could cause someone to fall and slip and potentially then knock Low
over a piece of equipment. To prevent this well try and film in a sheltered area.
Management factors (lack of Crew and Cast Medium We all need to communicate with each other so we know what each others Low
communication, co-operation, co- intentions are and if anyone is late or cant come we need to know so we can
ordination and competence) arrange a plan B. Ive collected a contact sheet so I have everyones numbers
and they should all have mine in case of an emergency.
Lack of food/water Crew and Cast Low Well all be bringing something to eat and drink along with us so that we can Low
hydrate and keep ourselves fed.
Costume Holly Adams Medium Holly will be wearing heels whilst shes acting and in our storyboard she falls over Medium
whilst wearing the heals. She could potentially break a bone or sprain her ankle if
were not careful. To prevent this from happening weve suggested she falls in
slow motion but when we come to edit the film well quicken the speed.
Camera Trolley Public, Cast and Crew Medium The Camera trolley has three legs which someone could potentially fall over and Low
injure themselves. To prevent this from happening were going to move the trolley
out of the way so that the public can past without falling over.
Uneven Floor Surface Crew and Cast Low The majority of the floor surface at the subway is tarmac and therefore even but Low
the actually subway themselves are slabbed and in some places uneven. With the
trolley especially the leg could get caught in a gap and dislodge potentially
dislodging the camera too and if the camera isnt attached properly the camera
could be damaged. To prevent this two people will carry the trolley to the set and
when the trolley is in use (camera attached) well get two people to check that
the camera is securely fastened.
