2. LCD Supervisor shall check the completeness of the CLR Form and the required
Attachments such as: 1. ROD Copy of the OCT or TCT
2. Certified True Copy of Latest Tax Declaration 3.
TIN of the Lessor 4. Government Issued ID of
the Lessor 5. Geodetic or Sketch Plan
3. A SPA must also be attached along with the CLR Form if the signatory of the lease
contract is not the owner of the property. The SPA must provide that the signatory is
authorize to sign the document and if also authorize to received the monthly rental, the
same shall be provided therein.
4. The LCD Supervisor shall prepare the lease contract based on the CLR form and
thereafter forward the contract to the Sales Manager to Cross Check the accuracy of the
contents of the Contract.
5. The Sales Manger, upon his discretion, may forward the drafted Lease Contract to the
prospect Lessor for their verificaion.
6. After the verification process the LCD Supervisor shall print the the drafted Lease
Contract for signing of the parties involved. If possible, the President of the company shall
sign the contract prior the Lessor.
7. A copy of the signed Lease Contract shall be attached by the Velox Sales Department in
the request for the Advance Payment and Deposit.
8. In cases where the signature of Company President cannot be obtain first, the
Accounting Department may ask for a certification from the LCD supervisor that the said
amount requested is the true amount based on the Lease Contract.
9. After finalization of the Lease Contract the VELOX Sale Executive Assitant shall advise
the Engineering Department of the perfected contract for them to proceed with the bidding
process.
10. Once the Lease Contract is signed by all parties involved the LCD Supervisor shall
ensure that the said contract will be notarized.
11. Once notarized the Lease Contract must be forwarded to the RDSO in charge for the
payment of the Documentary Stamp.
12. The LCD Supervisor shall keep a monitoring of the progress of the Lease Contract and
shall keep an organize file of all pertinent documents submitted therewith.

Phase 1 Lease Contract Preparation
Rationale
A completely filled-out CLR Form contains pertinent data needed for the
preparation of the lease contract.
The following documents are necessary to prove that the lessor is the true and
absolute owner of the property to be leased. Other requirements like Extra-
judicial partition or Deed of Donation may be requested depending on the
status of ownership of the Lessor
This is to allow the Lessor to review the contract before signing it.
The signing of the contract perfects the execution of the Lease Contract.
Moreover this is important to make the Lease Contract enforceable in the
court of law.
The signed contract is a proof that the requested amount is duly approved by
the Management.
LCD Supervisor
2. The Legal Supervisor shall prepare the Notice of Award to the Contractor granted with
the project.
3. The LCD Supervisor shall prepare the Construction Contract based on the Bill of
Materials forwarded by the Engineering Department.
4. Once the draft of the Construction Contract is ready it shall be forwarded to the
Contractor for verification of accuracy of the contents thereof.
5. After the verification process the LCD Supervisor shall prepare the documents for
signing and thereafter set the date of the contract signing .
6. After the signing of the contract, a copy of the signed contract shall be attached by the
Engineering Department in the request for the 30% Down Payment.
7. The LCD Supervisor shall ensure that the Construction Contract shall be notarized.
8. The Engineering Department shall advise the LCD Supervisor to prepare the Notice to
Proceed if the Contractor shall be ready to start the construction of the project.
9. The LCD Supervisor shall keep a monitoring of the progress of the Construction
Contract and shall keep an organize file of all pertinent documents submitted thereto.

Phase 2 Construction Contract Preparation
Rationale
The Bill of Material (BoM) is the end product of the Bidding process and shall
serve as the basis for the preparation of the Construction Contract
This serves as the official notice to the contractor that they get the project.
The BoM contains all the pertinent data for the preparation of the
Construction Contract.
The LCD Supervisor is the one tasked to facilitate the signing of the Contract.
The signed contract is a proof that the requested amount is duly approved by
the Management.
The date of the issuance of the Notice to Proceeds shall be the reckoning date
from which the counting of 90 days construction period starts.
