about the request of SIMON SABIANO (an inmate in your facility with prison no. L214-
P0252) for laboratory medical exam in the nearest hospital outside the Leyte Regional
Prison. Please find attached letter sent to your office about his request for your perusal.

Dear Sir:
We would like to inform you that Ms. Salvacion L. Sabiano sought legal
assistance from our office for the need of medical attention of his brother, SIMON
SABIANO an inmate with prison no, \214-P-0252 currently confined in your facility.
According to the client, Mr. Sabiano has already been checked by the infirmary at
your facility but has continually been experiencing the same symptoms without clear
diagnosis and treatment due to the absence of medical equipment necessary for a
full evaluation of his medical condition in your infirmary. The family is willing to
shoulder the medical expenses which wili be incurred should their brother be
permitted to undergo a general medical check-up in another hospital.
In connection with the above, we are respectfully inquiring on your end the
possibility of such request under your existing rules and further endorsing this matter
for your appropriate action.
