AFFIDAVIT

I, ALLAN Luci REANZARES, of legal age, Filipino, married, and a

resident of Pinagtongulan, Lipa City, after having been sworn to in
accordance with law hereby depose and state:

1. That, I am the father of ANDRIALYN Mamador REANZARES who

was born on January 13, 2013 at Poblacion, Bansud, Oriental
Mindoro;

2. That, my daughter Andrialyn has not been baptized in any church

due to personal and financial reasons;

3. That, I and my wife have finally decided that our daughter

Andrialyn be given the Sacrament of Baptism at San Vicente
Ferrer Parish Church at Barangay San Vicente, Lipa City this
coming December 25, 2016;

4. That, I am executing this affidavit to attest to the truthfulness of the

foregoing and for whatever legal purpose it may serve.
