Summary of Investigation
COMMUNITY PROVIDER
I. Initial Allegation(s):
It was reported that the person served requested to go to her room after
dinner and staff members, T.D. and W.F. would not let her. T.D. reportedly
yelled and fussed at the persons served and told one person served to shut
up. The person served then went to her room and was yelling and
screaming. It was reported that staff, W.F. hit the person served in the mouth
and head with a cloth with something hard inside of it. The person served
has no visible injuries and photos were taken of her mouth and head.
II. Conclusion(s):

III. Recommendation(s):
Per the DIDD Provider Manual, Chapter 18, 18.4.D.4, for Community Providers,
the summary of this investigation should be discussed with the involved service
recipient(s) within fifteen (15) business days of the receipt of the report. If a legal
representative has been appointed, they should be invited to participate in this
discussion. The space below has been provided for your convenience as a means by
