JOINT AFFIDAVIT OF DISCREPANCY IN NAME

WE, Maria de Joes AND Pablo Bartolo, both of legal age, Filipino
and with address at Malipayon Village, Roxas City, Capiz, under oath, depose
and state:

1. That, we know VILMA SANTOS, being a close friend of ours

for several years already;

2. That, in her record of birth at the Office of the Municipal Civil

Registrar in Ivisan, Capiz, it was entered therein that her first
name is BELMA;

3. That, said entry in her record of birth at the said office is

erroneous because the true and correct first name of Vilma
Odicta Delos Santos is VILMA and not BELMA;

4. That, the names VILMA SANTOS and BELMA SANTOS refer

to one and the same person;

5. That, we are executing this Affidavit to attest to the

truthfulness of the foregoing facts and to use the same for
whatever legal purpose it may serve.
