2/8/17 I created a parent survey in Google Forms for our Tier 4 1.1, 1.2, 1.4, 3.6, 3.7, 4.1, 1a, 1b, 1d, 3g, 4a, 5a, 5b, 5c, 6a, 6b,
students. The parents will be able to access and complete 4.2, 4.3, 5.1, 5.2, 5.3, 6.1 6c,
the survey at home. This will provide our Sped
department with strengths and weaknesses from a
parental point of view. We will use this data for the next
school year. [6 hours]

1. Briefly describe the field experience. What did you learn about technology coaching and technology leadership from completing this field
experience?

I was very honored and humbled to be asked to form and provide our school parents with the online parent survey that would provide the Sped department
with so much data for our upcoming school year. I used Google Forms to create the survey. The parents commented that they love this approach and hoped
that the school system would develop more surveys like this.

2. How did this learning relate to the knowledge (what must you know), skills (what must you be able to do) and dispositions (attitudes, beliefs,
enthusiasm) required of a technology facilitator or technology leader? (Refer to the standards you selected above. Use the language of the PSC
standards in your answer and reflect on all 3knowledge, skills, and dispositions.)

I felt that I had the knowledge, skills and disposition for the school to take on this project. First, I had to know how to work the technology effectively to
develop the online form. The parents needed to be able to click on the link provided on the school website to complete the survey. It was stated that my
infectious attitude to help and my belief that this form would be helpful to parents was the reason why they picked me to do this project.

3. Describe how this field experience impacted school improvement, faculty development or student learning at your school. How can the impact be
assessed?

The school will look at the data from this online survey to update and make changes to their Sped calendar and schedules. This will impact the schools
faculty and students greatly by making the school better than it already was. We will use this data to make suggestions on how to better improve our school.
