AFFIDAVIT OF LOSS

I, Sharmaine Edrosolano, Filipino, of legal age, married, and a resident of Brgy.Gustilo,

Lapaz Iloilo City, after being duly sworn in accordance with law, depose and state:

1. That I am a holder of a PRC ID bearing ID No. 064906 issued by Professional

Regulation Commission Region VI;

2. That I used to put the said PRC ID in my wallet;

3. That sometime on March 31, 2017,when I arrived home from work, I discovered that
my PRC ID was no longer in my wallet where I used to place it;

4. That despite diligent search and efforts to locate said document, I could not find it;

Thus, I execute this affidavit attest to the truth of the foregoing, to establish the facts and
circumstances of its loss and to support my application for the renewal of my professional
license as Certified Public Accountant.

IN WITNESS WHEREOF, I have hereunto set my hand this February 28, 2017 at Iloilo
