NOTES TO USERS:

This updated version of HSSE MS for BGP International is based upon the latest HSE-MS (F
version) of BGP Inc., published on January 20, 2012, as well as current relative standards and
guidelines from IAGC, OGP, ISM and SMS.
The requirements stated in this document are mandatory for compliance. Non-compliance is an
exception and is only authorized on case-by-case basis by senior management through
Management of Change procedures.
A controlled copy of the most current version of this document is held in the HSSE Department of
BGP International. Hence, before making reference to this document, it is the users responsibility to
ensure that any hard copy, or electronic copy, is current. For assistance, please contact the

Health, Safety & Environment Policy


This policy is the foundation of BGP Internationals HSSE Management System that
applies to all BGP International employees, contracted personnel and associates
involved in BGP International operations, and it is consistent with that of BGP.

The protection of health, safety and environment has always been and will continue to
be one of BGP Internationals highest priorities in its operation.

BGP International will set annual objectives and targets for continuous improvement in
HSE performance to eliminating or minimizing the health, safety and environmental risks
to the employees, contracted personnel and the citizens of the affected communities;
implement safe practices to strive for an incident-free workplace leading to ZERO
incident goal along with meeting all contractual, relevant regulatory and legislative
requirements where we operate.

BGP International will provide proper facilities, plant and equipment and maintain them
in a safe and secure condition. BGP International will also adopt and implement relevant
industry accepted HSE practices; provide suitable professional and job-specific training
for all employees and selected contractors; encourage openness and active participation
in HSE matters; and promote HSE awareness through regular campaign and education
programs.

Executive management and senior personnel are accountable for BGP Internationals
HSE performance, while a well defined line management structure enables all
employees to be responsible for fulfilling their duties.

Acceptable individual HSE performance is a key condition of continued employment with


BGP International, while excellent performance is officially recognized and rewarded on
regular basis.

SECURITY POLICY
BGP International is committed, with appropriate support from client and local
government, to provide security support and conduct its operations with minimum risk to
all personnel and company assets.

BGP International always abides by local laws and regulations with regards to the use of
firearms.

BGP International management in the country of operations must ensure that security
issues are properly addressed by implementing a country/project-specific security
management system including at least the following:

Identifying and evaluating threats, vulnerabilities and risks to company


personnel and assets.
Planning and implementing security measures, which are commensurate to the
predicted level of threat.
Establishing requirements for use of firearms and rules of engagement; not using
firearms, nor request its use to suppress peaceful demonstrations in the
communities, even if operational activities are disrupted.
Establishing a security and emergency plan to minimize the risk to personnel
and potential damage to company assets.
Training on best security practices to all employees, sub-contractors and
security personnel.
Monitoring security measures for continuous improvements as well as reviewing
of incidents for immediate corrective measures.
Adopting a No Resistance policy under apparent threats to life.
Liaison with law enforcement agencies, governments and to encourage peaceful
co-operation with local communities.
Every worker has a responsibility to observe the security principles and procedures
established by BGP International.

0 INTRODUCTION
0.1 Purpose of the HSSE MS Manual

The BGP International (BGPI) manual on Health, Safety, Security and Environment
Management System (HSSE MS) is one of the key documents governing the programs and
practices for its worldwide Land and Marine operations. Additionally, a bridging document
will be prepared and followed for seismic operations on each vessel between the vessels
SMS (safety management system) and this system to ensure a safe operation.
Individual crew and operating unit are required to demonstrate positive compliance with the
HSSE MS by having a project-specific or unit-specific HSSE Plan. All BGPI Senior
Managers, Vessel Masters, Party Chiefs, and Section Heads should be familiar with the
contents of the HSSE MS, and will be responsible for their own safety and health as well as
the safety and health of their subordinates. Therefore, they should integrate HSSE
programs and practices into their daily activities to foster a self-conscious safety culture, to
achieve the company and individual crew/ vessels annual HSSE goals and objectives, and
to comply with all the applicable HSSE laws, regulations and requirements as stipulated by
the local governments and contracts.
The HSSE plan for both land and vessel crew is the application of this system in its field
operations while sub-contractors are expected to have their own HSSE MS. If a sub-
contractor does not have an HSSE MS acceptable by BGPI, this system will be used by
BGPI to manage that party.
This HSSE MS provides processes to ensure that all parties within the same contract and
the client can work together without contradicting with each other.

0.2 Applicability of the HSSE MS Manual

This HSSE MS applies to the following personnel and properties for BGPI operations:
 All current employees.
 All properties or vessels owned or managed by BGPI.
 All land and marine operations.
 All sub-contractors, client representatives and other third parties who may be on a land
crew, company owned or operated vessels, or who may work on a project.

0.3 Elements of the HSSE MS

The updated HSSE MS is based on the key guidelines from IAGC, OGP, ISM, SMS, BGPs
current HSE-MS (Version F) as well as previous versions of BGPI HSE-MS. It includes
seven (7) elements as follows:
 Leadership and Commitment.
 Policies and Objectives.
 Organization, Responsibilities and Resources.

1 LEADERSHIP AND COMMITMENT

The management of BGPI is committed to implementing the HSSE MS for all the
operations. They will provide the leadership and resources necessary to ensure that the
HSSE MS is updated annually and practical to foster a proactive safety culture in the work
place, to meet the requirements of local governments and clients, and to enable the
company to achieve its annual HSSE goals and objectives. BGPI management is also
convinced that good HSSE performance will result in successful and profitable operations.
The leadership and its commitment are demonstrated in at least the following three areas:

1.1 Regulatory Compliance


BGPI will comply with applicable international and local laws and regulations wherever we
operate, for both land and marine operations. BGPI will conduct regular compliance audit
using its compliance assessment procedure to ensure compliance.

1.2 Zero Incident Culture


BGPI management will undertake practical and consistent actions to engender a zero
incident culture. Typical actions and programs are as follows:

 Include HSSE as an integral part of all business planning and project operations.
 Demonstrate personal exemplary behaviour by receiving required HSSE training and
conducting regular on-site HSSE visit/audit.
 Exercise visible leadership through consistent involvement in current HSSE issues.
 Ensure all employees accept individually-defined HSSE roles and responsibilities.
 Instil the conviction that HSSE is everyones responsibility.
 Encourage active participation and involvement by all employees in HSSE issues.
 Promote two-way communication throughout the company as well as with local
communities, clients and sub-contractors.

1.3 Incentive Program


BGPI management has established an incentive program to commend good HSSE
performance with reward and recognition on regular basis. Hence, both individual and
group HSSE performance will be assessed periodically based upon pre-determined HSSE
targets, as well as establishing the corresponding award and/or recognition from the results
of such appraisal.

2.2 HSSE Objectives and Targets

During the early part of first quarter of each year, BGPI management will set the annual
HSSE objectives and targets for the company with specific performance targets to be
determined and published by each Country management for its land seismic operations,
and for each vessel by the Marine Operations Management. The achievement of objectives
and targets will be reviewed on annual management review meeting.

3.1 Organization and Responsibilities

BGPIs headquarters is in Zhuozhou, a suburb of Beijing, where all the senior management
for both Land and Marine operations is located.
To facilitate operations worldwide, it has established regional and country based
organization for land operations as well as a separate Marine (deepwater) Operations
Management team managing all the vessel operations from Tanggu, a sea port of Tianjin,
about 120 km from Beijing.

3.1.1 Headquarters/Country/Marine Operations Management


BGPI has established an organizational structure based on its strategic objectives,
production, operational activities and businesses development. Hence a four-layer

3.1.5 Crew Management HSSE Function
The reference document below is a description of HSSE functions for key positions in a
typical Land and Vessel crew operation.

3.2 Training and Competency

All BGPI employees will receive specific job-related training according to BGPIs minimum
HSSE training requirements (referenced below), local regulations, project contract
requirements, as well as valid certificates required by STCW provisions and SMS for vessel
operations.
Sub-contractors are responsible for ensuring that their employees have received valid
certificates required by land operations, and comply with the STCW requirements plus other
relevant government and industry standards for Marine operations.
The crew Party Chief for each project has to ensure that this training has been conducted,
and records are kept such as appropriate certificates, if any, for certain types of workers --
drivers, welders, electricians, crane operators, fork lift operators, Marine workers, etc.

3.3 Regulatory and Contract Requirements

BGPI will comply with all applicable regulatory requirements of the local and national
governments where we operate, as well as the requirements stipulated in the contract
between BGPI and the client.
Changes or impending local regulatory requirements will be monitored and reported by the
management team at different level, including the Country Manager, land crew Party Chiefs
and vessel master. All identified non-compliance items will be recorded in the project/crew
RWP for timely corrective actions.
If the local regulatory standards are deemed as being less stringent than BGPI
requirements for the prevention of injury and/or environmental damage/pollution, BGPI will
maintain its own HSE standards in order to ensure a safe working environment.

Land

During the tender preparation stage, a joint effort by BGPI Legal affair, HSSE Department,
and land project team will review and document all relevant country and international laws,
codes of practice, and standards and guidelines. After receiving the contract award and
prior to the start of the project, contract requirements will be thoroughly reviewed by project

management to ensure compliance with its terms and conditions through preparation and
implementation of the Project HSSE Plan.

Marine
Like land operations, the Marine project team along with BGPI Legal, HSSE Department,
will review relevant local and national laws, as well as regulations on marine operations,
codes of practice, standards and guidelines during the bid tender stage. After receiving the
contract award and prior to the start of the project, contract requirements will be thoroughly
reviewed by project management to ensure compliance with its terms and conditions
through preparation and implementation of the Project HSSE Plan.

3.4 Sub-Contractors
All subcontractor personnel engaged by BGPI shall adopt BGPI standards and comply with
all applicable regulatory requirements of the country where we operate. Subcontractors will
be included in all HSSE work activities, programs and toolbox meetings to ensure that all
planned activities are reviewed, hazards identified and controls established for the
protection of health, safety and security of workers and the working environment.
Upon arrival on BGPI premises, all subcontractor personnel will receive HSSE orientation
training for either land project or vessel operation.

Land
Subcontractors for land operations are managed by the Party Chief and the Country/Project
management according to the BGPI subcontractor and supplier management procedures
referenced below.
As specific services, equipment, manpower provided by subcontractors vary from project to
project in both scope and duration, therefore, BGPI project management will exercise
extreme prudence in its selection and qualifying processes as well as supervision of
subcontractors to ensure all services, equipment, manpower provided by subcontractors
are acceptable by project management, and are planned, managed and incorporated into
the entire project HSSE/Operation execution plan.

Marine
Subcontractors for vessel operations are managed by the Party Chief of the seismic crew
and Master of the vessel according to the BGPI subcontractor and supplier management
procedures referenced below.
As specific services, equipment, manpower provided by subcontractors vary from project to
project and from vessel to vessel in both scope and duration, therefore, BGPI vessel and
project management will exercise extreme prudence in its selection and qualifying
processes as well as supervision of subcontractors to ensure all services, equipment,
manpower provided by subcontractors are acceptable by Marine project management and
the vessel Master, and are planned, managed and incorporated into the entire project
HSSE/Operation execution plan.

3.4.1 Personal Protection Equipment (PPE)

BGPI management is committed to providing all required PPE for workers, based upon the
results of risk assessments conducted for each land and marine project, as well as the
specific requirements of each contract and local government. This is to protect workers
from possible injuries and to mitigate the severity of potential risks as identified in the
project.
Minimum PPE requirements are listed in the reference document below for all types of
operations. Workers will be given instructions on the proper use and maintenance of their
PPE during the HSSE orientation tour with additional training for specific PPE required for
certain types of work.

3.5 Communication
It has been proven in the industry that regular dissemination, discussion, training and
reporting of HSSE information and issues, along with frequent feedback from workers have
increased workers HSSE awareness and enhanced their willingness to participate in
activities, leading to their eventual ownership of HSSE responsibilities. BGPI management
has been and will continue to devote much time and resources to foster and achieve such a
proactive HSSE culture.
Additionally, frequent communications with local communities, related governmental
agencies and other interested parties will facilitate the operations. BGPIs programs as
listed below will be used for such endeavour.

3.5.1 HSSE Briefing

BGPI requires that all visitors receive a briefing upon their arrival on BGPIs premises. The
briefing is to be conducted by one member of the projects senior management and the
following contents should be included as a minimum:

 Site introduction.
 Major hazards communication.
 Emergency response plans.
 Other information upon visitors request.

3.5.2 Orientation

BGPI requires that all new employees will receive a full HSSE orientation conducted by the
HSSE Department of the crew soon after their arrival. The following are the minimum topics
that should be included in an HSSE orientation:

 Company HSSE policies.


 Site specific hazard communication.
 Fire fighting and evacuation plan.
 Emergency response procedures.

 Job and associated hazards.
 Job description.

3.5.3 HSSE Meeting

BGPI management recognizes the importance of HSSE meetings in promoting and


fostering a proactive HSSE culture. Hence, meeting requirements have been set to ensure
a platform for frequent two-way communication. A land or vessel crew will conduct the
following types of meeting according to different time period;

 Toolbox (hand-over) meeting--daily.


 Land crew sectional HSSE meeting--weekly.
 Crew HSSE Committee meeting--once a month.
 Whole crew meeting--held as required.

Additionally, Company/Country/Marine Operations Management has established HSSE


meeting schedule as follows:

 Land --Country Management--Quarterly.


 Marine Operations Management--Quarterly.
 BGPI Company HSSE Committee meeting--Quarterly.

Except the daily toolbox meeting, all the meetings listed above will generate formal meeting
minutes which will be distributed to all relevant personnel.
The toolbox meeting is extremely vital to the safe operation of a project as well as a golden
opportunity for the Party Chief, Master, Section Supervisor to address key HSSE issues
and daily activities to their workers. Particular emphasis should be given to identified risks
of the day, along with controls and mitigating measures.
Other meetings will focus on a review of HSSE issues, including any incident reports or
investigations as well as HSSE performance. All participants in the meetings are
encouraged to actively participate in the discussion.

3.5.4 Routine Reporting

Daily HSSE Report

Each HSE Advisor for land or vessel crew is responsible for collecting a specified set of
HSSE statistics on daily basis based upon BGPI and contract requirements. This data will
be kept by the respective land and vessel operations project management team and is not
reported to the Headquarters HSSE Department. Refer to the reference document below

Each land or vessel crew HSE Advisor is responsible for preparing and submitting this
report to the Headquarters HSSE Department on a weekly basis. Specific activities and

Monthly HSSE Performance Analysis (Country Level/ Marine Operations)

Country HSE manager along with the Country manager will be responsible for conducting a
thorough review of HSSE performance for all the land crews within that Country on monthly
basis as described in section 7.3.2 of this manual. The result will be sent to the BGPI HSSE
Department for review by senior management.
The Marine Operations manager and the Marine HSE manager will be responsible for
reviewing the HSSE performances of all the vessels on monthly basis as described in
section 7.3.3 of this manual. The result will be sent to the BGPI HSSE Department for
review by senior management.

Others--Alerts, Awards, Recognition, etc.

Safety alerts from BGPI, other seismic companies and oil companies will be reviewed with
affected workers at HSSE meetings and to share the lessons learned. These alerts are in
addition to the information and reports communicated regularly to workers.
BGPIs incentive program provides individual and group awards/recognitions for excellent
HSSE performance on regular basis. These awards/recognitions will be based upon pre-
established criteria set by each land crew with its Country Management, and vessel crew
with its Marine Operations Management.

3.6 Community and Public Relations


BGPI senior management along with country and crew/vessel management encourages
feedbacks on HSSE management and performance for its operations from employees, sub-
contractors, clients, relevant local and national governmental agencies where BGPI
operates.
The Community/Public Relations coordinator, when available for the project, will help the
Country Manager along with Project Manager and Vessel Master/Party Chief to receive
such feedbacks and prepare for appropriate and timely responses.
The project management team are also responsible for conducting education and
consultation programs to help the local community better understand our operations, and to
seek their support, if possible, for permit application, logistics and security assistance, etc.
Furthermore, senior management, along with Departments of Marketing, Operations,
Equipment and Resources, and HSSE at BGPI Headquarters are within the above

communication loop to ensure each crew/vessel has such programs in place, and they are
maintained and implemented effectively.
All the above feedbacks, responses and programs will be documented and kept on file by
the relevant offices of Country Manager/Party Chief/Vessel Master. These documents are
available for review during audits by senior management, client, and BGPI hired third-party
auditor as well as HSSE Department of BGPI Headquarters.

3.7 Documentation
Relevant documents listed in the HSSE MS as referenced below are kept for record with
specific retention period as well as periodic maintenance of the system.
Additionally, a list of key documents for each land or vessel crew is controlled and
maintained by the crew HSSE Department.

4 RISK MANAGEMENT
Risk Management is the basis for proactively managing Health, Safety, Security and
Environmental issues for BGPI operations.
Hazard identification, assessment and control/mitigating measures are implemented across
all activities undertaken by BGPI. All significant hazards are controlled and/or mitigated to
ensure that potential risks are reduced to as low as reasonably practicable (ALARP) level.
Assessment of significant risks for Health, Safety, Environment and Security for a specific
project is routinely conducted and documented. If an assessment has identified control
gaps within the BGPI system, these deficiencies are recorded in the projects or crews
RWP system for appropriate improvement actions to eliminate such gaps.

4.1 Hazard Identification/Assessment/Control

Each land and vessel crew will maintain a project-specific Hazard Register along with their
control measures which will be included in the risk management plan section of the
Project HSSE Plan.
Project-specific HSSE hazards/risks identification activities will be conducted during project
scouting, planning and mobilization stages by an experienced team of employees. This will
become a daily activity for all workers once operation is commenced.
To identify and eliminate hazards and risks caused by unsafe behaviour of workers, an
HSE Observation Card Program has been introduced for all crews to implement. Specific
training for this program has also been designed and conducted on continuous basis for
Headquaters and crew management. See reference below in Table 2 for contents of an
observation card.

4.2 Risk Assessment & Control

4.2.1 Risk Assessment


Project specific risks are identified during the early stages of scouting, planning as well as
mobilization stages for both land and marine operations by project team members, local
experienced workers along with on-site client representatives.
The Party Chief or Vessel Master for each project is responsible for organizing a team of
experienced personnel to conduct a thorough assessment of all identified project-specific
risks using the latest BGPI HSE Risk Matrix (Table 3 below) to determine the potential
severity and frequency of each hazard/risk. A specific risk matrix for security assessment is
also used for each project. If the result of an individual assessment falls within the Red
Zone of the Risk Matrix, control measures must be implemented to reduce its potential risks
so that the residual risk falls within at least the Yellow Zone or preferably in the Blue Zone if
reasonably possible. This is documented in the Project HSSE Plan and is updated regularly
as new risks are encountered.

4.2.2 Risk Control


BGPI will eliminate, reduce and/or control all identified hazards/risks in accordance with the
companys written standards and accumulated expertise by either reducing its probability of
occurrence or reducing its effects or both. This may be achieved by one or a combination of
the following steps to reach ALARP (as low as reasonably practicable) level:
Eliminate, Substitute, Reduce, Isolate, Control, Protect and Behavior.
After the appropriate control measures have been selected, the residual risk has to be re-
assessed using the Risk Matrix to ensure that the remaining risk is at ALARP level.
All identified hazards/risks are controlled and/or mitigated by the following procedures for
both land and marine crews.

 JSA Procedure
Job Safety Analysis (JSA) is conducted routinely by land crew and vessel crews in order to
systematically review a particular task. The completed JSA is addressed during toolbox
meetings in order to ensure that all relevant workers are very familiar with the identified
hazards/risks as well as control measures to be taken. Supervisors and foremen are
responsible for implementing this procedure.

The supervisor or foremen of a small group is responsible for conducting an on-site
assessment with the workers right before starting an operation. Emphasis is on the risks
and control measures as discussed in the JSA above. Additionally, workers are encouraged
to report any risks/hazards not identified in the JSA, and report to the supervisor or foreman
for his decision to stop the work temporary until such risk/hazard has been controlled.

Each and every chemical and hazardous material used by BGPI operations requires an
individual MSDS. Supervisors will train workers who handle and use these materials in
accordance with instructions from the MSDS as well as health and emergency measures.

To prevent injury and minimize risks for a specific project, crew management for both land
and marine projects will prepare a list of permitted and not-permitted operations, and
review them with client representatives for their agreement. Upon approval, this list will be
explained to all workers and require them to follow all the details for the duration of the
project.

Land

Since BGPI operates in many parts of the world with so many clients, the content of each
MOPO will vary from project to project as detailed in the project specific HSSE Plan.

Marine

Like the land operations, BGPI vessels work around the world for many clients. Therefore,
the activities in each MOPO will vary from project to project as stated in the project-specific
HSSE Plan. However, the basic activities in a standard MOPO for Marine project will
remain the same as stated in the referenced document.

international standards, mostly European and US OSHA, for all land and marine operations.

 Simultaneous Operations -- Marine


This procedure is established to ensure safe operations and coordination among all parties
working in a very limited space at the same time. All workers must be familiar with this
procedure as well as the Safety Management System ( SMS) of the vessel.

4.3 Permit to Work (PTW)

A Permit to Work (PTW) system is implemented to include at least the following work
activities for both BGPI personnel and its subcontractors:

 Hot Work welding, oxy cutting, grinding, etc. (any work where heat or sparks are
generated). A PTW is required only if this work is conducted outside the designated
Mechanical Workshop.
 Working At Height any work where a worker is required to be at more than 2 meters
above a protected work surface must follow the procedure as listed below.
 Confined Space Entry any entry into a space where there may be oxygen deficiency

must follow the procedure as listed below.
 Electrical Work any work associated with the main power supply and major electrical
system.
 Ground Disturbance---any work that involves manmade cut, cavity, trench or
depression in the earths surface formed by earth removal must follow the procedure as
listed below.
 The Lock-out/Tag-out (LOTO) procedure is a key program of PTW that has to be
implemented for all work on electrical, mechanical, air and hydraulic
equipment/systems where energy sources have to be depleted and/or isolated before
work can commence.

The PTW system is managed by the Party Chief of Land crew or the Master of the vessel,
only the Party Chief/Master or his designated alternate can approve a PTW.

4.4 Implementation of Control Measures


The section supervisors are responsible for informing the individual group of the specific
risks and control measures before they begin each task, in addition to addressing the risks
and control measures for that day in the daily toolbox meetings.
Each identified hazard and its control measures are implemented and recorded in the
Remedial Work Plan (RWP) of a project. The HSE Advisor of the land and vessel seismic
crew is responsible for updating the RWP and communicating regularly with the crew and
upper management. The RWP is also used to track all deficiencies identified by audits,
visits, inspections, drill, compliance assessment and incident investigations of a project.

4.5 Management of Change

A Management of Change procedure has been developed and implemented to ensure all
significant changes to any aspect of the personnel/operation are reviewed and approved
before the change is implemented. The review process includes a risk analysis which
ensures that the proposed change does not introduce additional hazards, that processes
and procedures for managing change are applied, and that HSSE standards and
requirements are not compromised by the change. If new hazards are identified, additional
controls will be implemented to reduce them to ALARP level.

The management of change (MOC) process consists of four (4) steps:

 Plan and Procedures of land operation


Operations such as line clearance, drilling, shooting, ground disturbance, vehicle movement,
camp operation; along with the wastes generated by the crew from base camp and
operating areas are controlled and disposed of according to the project Environmental
Management Plan in order to minimize its impact on the environment.

 Plan and Procedures of marine operation


For each vessel operation, an environmental management plan using the BGPI
Environmental Guidelines and reference document below will be prepared and
implemented to monitor and track its operations such as deploying and retrieving of
streamers and/or cables and air gun operations, etc. along with their impact on sensitive
areas such as fishery and presence of marine mammals, if any, as identified by the
environmental assessment report, as well as the waste generated, air emissions and water
discharges, noise, etc. in order to control and minimize their impact on the environment.
Specifically, all wastes generated on board each vessel are controlled and disposed of in
accordance with the vessels waste management procedure and in compliance with the
MARPOL requirements.
If cetaceans are present in the operating areas as identified by the environmental
assessment report, then a cetacean monitoring and acoustic minimization program will be
implemented by referencing the documents below, the current industry standards, and as
stipulated by contract.

5.1.3 Occupational Health & Medical Management

The BGPIs Occupational Health & Medical Management Guidelines referenced below is
used for all land and vessel operations.
BGPI management is committed to the goal of no harm to people with a healthy workforce
and working environment. Therefore, the BGPI Occupational Health & Medical
Management Guidelines has been issued for all planning purposes for occupational health
& medical management. Additionally, all relevant requirements from the contract and local

government will be included in the project-specific HSSE plan for both land and vessel crew
to implement.
An occupational health risk assessment will be conducted using the guidelines referenced
below, if necessary, by medical personnel from either BGPI or a qualified third party for a
specific project. Recommendations from this assessment will be incorporated into the
Project HSSE Plan for implementation.
All locally hired workers will undergo BGPI medical test prior to official employment. The
project crews medical staff will advise for appropriate actions for prevalent diseases in the
project area as identified in the assessment report.
All BGPI employees and subcontractors are required to pass physical fitness medical exam
conducted either by a BGPI doctor or a third-party medical professional. Additionally, some
positions are also required to obtain valid certificates if required by international health
standards and/or equivalent governmental standards.

5.1.4 Security Management

BGPI management is committed to a secure working environment. Therefore a Security


Management Guide referenced below has been issued for all operations. In addition, all
requirements by the contract and local government, if any, will be included in the project-
specific HSSE plan for the land or vessel crew to implement.

If requested by the Country Management, a security risk assessment will be conducted by
an in-house or a qualified third-party consultant before project mobilization.
Recommendations from this assessment will be incorporated into the Project HSSE Plan
for implementation.

Marine
Each vessel will implement its own security plan and procedures. In addition and if
necessary, an assessment for security issues for specific land docking ports and working
areas will be conducted by an in-house or a qualified third-party consultant for appropriate
control measures. Recommendations will be included in the vessels HSSE plan for
implementation.

5.2 HSSE Plan

5.2.1 Project HSSE Pre-startup Planning

The latest OGP Report #432 is used as guidelines to prepare a project specific HSSE plan.

The first stage in the planning process for a project is the identification and listing of all the
operations HSSE activities by the project team members to ensure a complete planning
process along with appropriate actions to facilitate a safe and efficient operation.
Then the project team members will review, discuss, prepare and finalize the Project HSSE
Plan for the project, review it with on-site client representative before commencing an
operation.
After review and approval of the Project HSSE Plan by the client, all project/crew
management (Country/Project managers/Party Chiefs/supervisors) as well as other and
local workers will receive extensive pre-startup HSSE training based upon the Training
Matrix specified in the Project HSSE Plan. This training will be conducted by either internal
or external experts.

5.2.2 HSSE Plan

The HSSE plan is a project-specific plan for both land and Marine operations which is
prepared by using the BGPI HSSE Plan template(using OGP #432 as reference) that
contains the following key sections:

Using the above referenced template to prepare a project-specific HSSE plan, the project
team has to review and incorporate all project-specific risks/control measures, training plan
and requirements, journey management plan, international maritime requirements, etc. plus
stipulated requirements in the contract with the client, as well as the local governmental
requirements on marine activities.
A bridging document is prepared, if needed or required, (to be included in the Project HSSE
Plan) to include the differences between the HSSE requirements of the contract and that of
BGPIs HSSE MS, as well as the requirements of the local government where we operate.

5.3 Emergency Response Planning


BGPI requires that an emergency response plan as referenced below be prepared and
implemented for all BGPI operations that includes all possible emergency situations for that
specific project while complying with the local, national and contract requirements. This
plan is part of the Project HSSE Plan. Additionally, each land operation crew will have an
emergency evacuation plan.
The plan is to ensure adequate, appropriate and rapid response to all possible emergency
situations on site, to minimize personal injury, environmental impact and property loss, and
to provide additional support to the land and marine crews if necessary to prevent an
incident from escalating into an emergency situation.

The emergency response plan is based on the following goals:

 Saving human life.


 Moving to a safer place.
 Saving assets - (without endangering personnel).
 Protecting the environment - (without endangering personnel).

Emergency planning is divided into three phases:

5.4 Safe Working Procedures

BGPI has established a comprehensive set of Safe Working Procedures for all seismic
activities for both land and vessel operations to ensure that workers are following the
procedures to safeguard against injury, health, environmental impacts and associated risks.

As stated in section 5.2.2, all workers have to receive project-specific HSSE training before

Many copies of the Safety Training Manual in accordance with requirements of SOLAS is
on board each vessel. It is mandatory that all crew members (including the seismic crew)
are very familiar with this manual as well as safety equipment and procedures on board the
vessel.
Additionally, the seismic crew on the vessel shall receive training on vessel-specific seismic
operating procedures as listed below to ensure a safe operation.
The seismic crew on the vessel shall receive vessel-specific training on the seismic-related
sections of vessel procedures listed below to ensure a safe operation.

Relevant safe working procedures have been established for at least the following areas:

 Vessel Operations, including:


 Bridge Procedures
 Engine Room Procedures
 Deck Procedures
 Seismic Operating Procedures
 Safe Management System (SMS) for vessel
 Helicopter Operating Procedure (See reference document below for detail)

The Vessel Master and the Party Chief are responsible for ensuring that all HSSE
procedures associated with this operation are reviewed at least annually, and updated as
required by using the Management of Change procedures. This review will be documented.

6 IMPLEMENTATION AND MONITORING

BGPIs HSSE MS is implemented in its land and marine operations through a project-
specific HSSE Plan. This plan provides key instructions and procedures to execute
operations safely and successfully, and to ensure that:

 All workers are physically fit and trained to work safely.


 All the hazards/risks in the work environment have been identified, controlled and
monitored for corrective actions.
 The crew meets the requirements of both the contract and local government.
 The required safe working procedures are adequate used and followed.
 The equipment and facilities used are regularly inspected and maintained.
 Appropriate HSSE emergency response plans and measures are in place.
 Project-specific Environmental Management Plan (EMP) is in place with its actions
implemented.
 The required documents/records are kept to facilitate the operation.

6.1 Implementation of Project HSSE Plan

The following activities are undertaken by each land and vessel crew as they implement
their Project HSSE Plan.

6.1.1 Non-Compliance and Corrective Action Tracking System

All incidents of non-compliance that include deficiencies from management visits, client
audits/inspections, internal and/or third-party audit, and inspections are to be recorded in
the project/crews Remedial Work Plan (RWP) along with their required remedial actions.
The RWP for each land and vessel crew is submitted to BGPI Headquarters HSSE
Department on a weekly basis for management review and follow up.

6.1.2 Risk Management Program

A toolbox meeting is held each morning for land operations or shift change hand-over
meeting for vessel operations conducted by the Party Chief and/or crew supervisors to
address and emphasize at least the following:

 Daily activities and the specific HSSE risks for that day as identified and recorded.
 Review specific control measures for the specific risks.
 Ensure all workers understand and are ready to take the appropriate control measures.

A weekly HSSE meeting is also held for the project/crew management for both land and
vessel operations to address at least the following issues:

 HSSE issues of last week with possible solutions.


 Plan for HSSE activities for the following week.
 Specific issues from operations.

6.1.3 Emergency Drills and Exercises

Scheduled emergency response drills and full scale exercises for first aid, fire, evacuation
and Medevac (plus project-specific emergency) are planned and conducted, with
observations noted and comments documented for improvement.
Emergency Response Team members for both land and vessel crew will conduct more
frequent drills and exercises among themselves to ensure they are ready to respond to an
emergency.

6.1.4 Inspection and Maintenance Program -- Facilities, Equipment and Vessels

Operating facilities along with all equipment for land projects and vessels will receive
periodic inspections by crew management to ensure they are in safe operating condition. A
preventive maintenance program is also established and implemented for both land project
and vessels. The Chief Mechanic on the crew or vessel will be responsible for ensuring that
a record is kept for all inspections and maintenance, with deficiencies noted in the projects
RWP for immediate action.

6.1.5 Routine HSSE Reporting

Daily and weekly HSSE reports are required for both land and Marine operations. It is the
responsibility of the HSE advisor for each project to ensure that these reports are prepared
and submitted to the designated internal personnel/department and client representative.

6.2 Asset Integrity

All BGPI owned assets will be designed, procured, constructed, operated and maintained
according to the guidelines listed below, the local laws and regulations as well as contract
requirements, to ensure HSSE considerations are incorporated in all relevant aspects to
warrant their integrity.
A preventative maintenance program shall be included and reguarly updated, including as a
minimum all safety critical assest, products and equipment.

All of the above indicators will be recorded in the projects Daily/Weekly HSSE Performance
Monitoring report, and are used as a basis for continuous improvement.

6.4 Operating Records

HSSE operating records for both land and vessel operations are essential to the success of
a project; they also serve as a collection of experiences for our workers to prepare and
execute the next project. In addition to the required data to be recorded by BGPI, additional
requests by the client and/or local government will be included in this record.
The crew HSSE Department is responsible for keeping this data updated on regular basis
with a complete file at the end of the project.

6.5 Incidents, Near-Misses and Unsafe Situations

6.5.1 Classification

Incidents are defined and classified according to OGP, IAGC, and ISM system guidelines
as follows. Refer to the referenced documents below for details.

Cat. Definition
FAT Fatality. An accident results in the loss of life of worker.
LWC Lost Work Day Case.
RWC Restricted Work Case. The injured worker is unable to conduct normal
duty at the regular working hours on the next day following the accident,
but able to work in a different capacity.
MTC Medical Treatment Case. Needs medical/paramedic care, but fit for work.
FAC First Aid Case. Minor injury requiring first aid treatment (as defined in the
referenced document below) and fit for work.
DAM Damage incident involving primarily property loss.
ENV Environmental incident.
SEC Security incident.
HiPo High Potential. An incident that could have easily resulted in Severity
consequence level of 4 or 5 on the BGPIs Risk Assessment Matrix
(RAM).

6.5.2 Reporting

The reporting of an incident is managed under a No Blame culture. BGPI encourages the
reporting of all incidents in order to foster a proactive safety culture, it will also facilitate the
construction of a realistic company-specific incident profile along with a trend analysis and
prediction database for appropriate control measures to prevent future injury, damage or
environmental incidents.

6.5.3 Investigation

The most important reason for conducting an investigation is to prevent recurrence of the
same or similar accident according to the procedure referenced below.
The investigation report will identify all causes (direct, indirect and root) of the accident, and
provide recommendations for corrective actions to prevent reoccurrence of the accident. All
corrective actions will be entered into the projects Remedial Work Plan (RWP).

6.6 Clients and Products

BGPIs Marketing department is the key contact with clients for communication and
feedbacks on the overall performance of both land and vessel operations.

Country/Project manager and/or Party Chief for land and vessel operation will be the
contacts with the client representative on regular basis to review HSSE performance, to
coordinate all project activities and services, and to ensure all HSSE related requests,
issues and concerns are addressed on timely basis.

7 AUDIT AND REVIEW


7.1 HSSE Audit & Inspection

HSSE audit/inspection and review are conducted for the following key reasons and
according to the procedures reference below:

 To ascertain that the Project HSSE Plan is in place, and if it is being implemented and
functioning efficiently.
 To ensure compliance with current policies, local regulatory and client contract
requirements, as well as the BGPIs HSSE Management System.
 To assist project/vessel management in the identification of deficient areas during
implementation of the Project HSSE Plan, and to provide recommendations for
improvement, hence enhancing the crews HSSE performance.
 To decipher if the Project HSSE Plan and other applicable HSSE standards and
procedures used are adequate for the specific operations; if not adequate, then
recommendations for immediate actions will follow.

Audit: It is an independent examination and assessment of how the HSSE Management


System is implemented and documented throughout the organization. Specifically, how the
HSSE Plan for a specific project or vessel operation has been functioning over a period of
time.
Inspection: It is a physical observation and/or examination of all or selected equipment or
facilities or a vessel, etc. used for a project. Inspection checklists are used for both land and
vessel operations.

7.1.1 Internal Audit Program and Procedures

BGPI has published an audit procedure guideline as referenced below for its operations.
This is a program to standardize the quality, scope, content, frequency, activity and
reporting of several types of audit used by the operations.
Trained and experienced auditors are employed to ensure a thorough review and
assessment of all aspects of the operation. A formal audit report shall be prepared at the
conclusion of an audit, and submitted to BGPI management, project management and the
HSE Manager for review and follow up actions. All the deficiencies and recommendations
from this audit report are recorded in the projects RWP by the crew HSSE Department.

7.1.2 Third-Party Audit

This is an audit conducted by a reputable consulting firm to assess how HSSE MS is


functioning in a specific project or vessel operation. Before conducting such an audit, the
BGPI HSSE Department along with the senior management of operations for either land or
marine crew will decide on the following items with the consultant:

 Which project to audit -- BGPI management will decide.
 Detailed scope and agenda -- BGPI management will decide.
 Basis of the audit -- using BGPI HSSE MS and Project HSSE Plan.
 HSSE documents that the consultant need to review--consultant to provide advice.
 Project personnel and/or workers that the consultant needs to talk to during the audit.
 Date and duration -- BGPI management / consultant will decide.
 Audit team members -- Advised by consultant and approved by BGPI management.
 Logistics arrangements and PPE requirements--Provided by BGPI Project
management.
 Contents of the audit report and its due date-- to be set by BGPI management.

An audit report is prepared by the consultant at the end of such audit and submitted to
BGPI management within a specified time frame for review and planning for improvement.
Detailed deficiencies and recommendations in the report are noted in the projects RWP by
the crew HSSE Department for follow up action.

7.1.3 Client Audit

Client may conduct an audit whenever notification of such activity is received by project
management. Before conducting such an audit, the BGPI project management has to
discuss with the client representative to ascertain the following items:

 Detailed scope, schedule and agenda.


 Basis of audit -- using the projects HSSE Plan and/or the client companys HSSE MS
system.
 HSSE documents that the client plans to review.
 Project personnel and/or workers that the client plans to talk to, if any, during the audit.
 Field or vessel operations to visit.
 Audit team members.
 Logistics arrangements and PPE requirements.
 Audit report specific information the project team needs to provide to the client to
complete this report.
 Sharing of audit findings -- set the time frame the project team will receive the final
report.

7.2 Compliance Assessment

A regular compliance assessment will be conducted by the Project /Crew management


team with the help of Legal Department and third-party consultants, the time frame for the
assessment may typically be as follows:

 For the project lasting over one calendar year, the assessment shall be carried out
annually during the months of December.
 For the project lasting less than one calendar year, the assessment shall be carried out
at the end of such project.

Furthermore, the compliance assessment may be conducted under the following


circumstances:

 After a major HSSE incident.
 Changes in local laws, regulations, standards or contract requirement.
 Major findings from HSSE audit.

The compliance assessment procedure below will be followed with all identified deficiencies
recorded in the project/crew RWP for timely corrective actions.

7.3 Management Review

7.3.1 Headquarters Review

The BGPI HSSE Committee will conduct an annual formal review and update of the HSSE
Management System Manual and HSSE performance in conjunction with the HSSE
Department according to the review procedures referenced below. BGPI President along
with HSSE Manager will organize this review.
Additionally, the following activities and documents are reviewed quarterly during the
Company HSSE Committee meeting to ensure all operations are operating in compliance
with the specific regulatory and contractual requirements, and in accordance with the HSSE
MS through the individual Project HSSE Plan;

 HSSE performance for all operations and trend analysis.


 Status of communication and feedbacks from client company headquarters and issues
to be addressed from client audits/inspections.
 Specific HSSE issues of current and future projects.
 Specific legal requirements from projects and/or contracts that cannot be resolved by
Country/Marine Operations management.
 All major accidents along with status of implementation of remedial measures.
 Major RWP items that require attention.
 Initiatives and new programs from IAGC, OGP and other relevant industry associations
that might affect BGPI operations.
 Status of implementation of Project HSSE Plan.
 This review is documented and distributed to all Headquarters Departments and
Country/Marine Operations Management.

7.3.2 Country/Marine Management Review

The Country/Marine Management shall conduct their HSSE performance review on


quarterly basis. The Country manager and Marine management will organize this review.
The following activities and documents will be reviewed quarterly to ensure that they are
operating within the regulatory and contractual requirements, and provide needed support
to all projects within that Country or all vessels to ensure a safe and successful operation;

 Communication with the country client representative, international maritime
organization, government and other parties, local communities, etc.
 Status of proposed actions/requests by each project management.
 Monthly HSSE performance and RWP statistics submitted by all projects within that
Country or vessel operations, along with their trend and remedial measures.
 Specific issues raised by each project management.
 Status of implementations from audit/inspection/accident reports.
 Specific Country/International maritime regulatory issues, if any.
 Specific issues of contract requirements raised by project management.

This review will be reported and documented for further review by BGPI headquarters and
distribution to all the project management within the organization of country/vessel
operations.

7.3.3 Land and Vessel Crew Review

On monthly basis, the vessel Master and the Party Chief for seismic project will organize a
review meeting for their operation. Participation by on board or land client representative is
welcome but not necessary.
The following activities and documents will be reviewed by the crew management for land
and vessel operations to ensure that they are operating in accordance with their Project
HSSE Plan, vessel SMS, contract requirements, local government and international
maritime guidelines. This review will be documented for further review by the
Country/Marine Management.

 Daily and Weekly HSSE performance.


 Project RWP status.
 Major HSSE issues for the week or month.
 Identify and request support needed from Country management.
 Communication with project client representative, local community and other relevant
parties, with proposed corresponding actions.
 Internal audit/inspection results.
 Unsafe act and unsafe condition reports.
 Project HSSE Performance Trend Analysis.
 All incident reports, if any, along with the status of all action items from the report.
 Specific contract HSSE requirements, if needed.
 Specific Safety Working Procedures for land operations, if needed.
 Specific Vessel Safety Procedures (VSP) -- related to seismic operations only, if
needed.
 Vessel Operating Procedures (VOP) --related to seismic operations only, if needed.
 HSSE Meeting minutes for the project.
 Review report/comments from Country/Marine Operations Management.

7.3.4 HSSE Review Procedures

The referenced procedures below are used to conduct a systematic review of HSSE
performance and related subjects. In addition, the key items listed below must be decided
during the planning for such a meeting at the project level as well as at the level of

Country/Marine Operations Management. The BGPI HSSE committee has already
established its own procedures:

 Prepare an agenda with date and time, items to be reviewed as well as the party
responsible for preparing and presenting each item in the meeting.
 Specify any pending HSSE issues that need to be resolved--for each project or vessel.
 Select members for the review meeting.
 Obtain specific outcome from the review.

At the end of all review meetings, all findings shall be addressed along with determination
of appropriate remedial actions for corrections and improvement, these are recorded in the
RWP.

7.4 Remedial Work Plan ( RWP)

RWP is a tracking system to record all deficiencies identified from all management visits,
audits, inspections, drills, and compliance assessment before and during the operation. The
responsible party is designated for the timely reporting and closure of all items in the RWP.

7.4.1 Land --Trend Analysis and Effectiveness of Program

The BGPI HSSE Department is responsible for analyzing the RWPs for all projects for an
in-depth understanding of what had happened, as well as noting the conspicuous and
repeated trend of deficient areas on monthly, quarterly and yearly basis for BGPI operation.
Effectiveness of the RWP system depends entirely on how each action item on the
project/crew RWP list is closed-out as follows:

 Conciseness and clarity of description of each action item.


 The thoroughness of assessment and control/mitigation measures.
 Ensure each action item is indeed closed properly when it is closed on the RWP
spreadsheet.
 Outstanding action items on the RWP list are reviewed with urgency for prompt actions
in the crew weekly HSSE meeting.
 The crew management familiar with the items on RWP list, and are actively assisting
the responsible parties to close out the action items.

7.4.2 Marine -- Trend Analysis and Effectiveness of Program

Results of HSSE performance analysis from the BGPI HSSE Department for each vessel
operation will be sent back to the Marine management as well as the vessel management
for their review and discussion for specific controls and/or mitigation measures.

In addition to the description listed under Section 7.4.1 for land operations above, the
effectiveness of the RWP program for each vessel operation also depends on the following
specific activities for its success:

 How well the SMS system for the vessel is functioning with the seismic crews Project
HSSE Plan.
 How well the seismic and vessel crew coordinate and communicate with each other.
 How well the seismic and the vessel crew understand each others working procedures
and work as a team to close out the vessels action items.
