Introduction
Summary
CNAbiera freelance service team will be designing and developing the Korean plastic surgery review,
forums and blog website.
Web project will be using Wordpress CMS for ease of use.

Vendor will attempt to complete the website within 3-4 months if scope of work is followed
without any additional changes.
Aside Website will consist of 3 main features:
o Review Functions
o Basic Blog Functions
o Basic Forum Functions
Project will commence upon receipt of the Signed Statement of Work and deposit.
Objectives
The objective of this project is to develop and design a wordpress website which caters to reviews,
forums and blogging function for Korean Plastic Surgery services.
The vendor will attempt to complete the scope of work within 3-4 months as per clients request but is
not guaranteed if some unforeseen challenges occurs within the development period.
Scope of Work
Included:
The scope of work for this project is comprised of the following deliverables:
1. Web development of a website based on a Wordpress CMS.
2. Implementation of basic pages such as:

o Home Page
o About Page
o Service page
o Contact us Page
3. [Review feature] Clinic Listing Page with a dynamic arrangement and clinic listing column with
the following elements:
o Name & corporate logo of the clinic. (Linkable to clinic profile panel)
o Clinic Popularity Numeric Ranking (1st, 2nd, 3rd, etc.) Based on verified reviews archived
under the clinic ONLY.

archived will always display at the bottom of the previous review on the same

6. [Forums Feature] Website will have a basic forum feature to cater to member forums.
o New Clinic Submission Form
Clinic name field, allow visitor to type the clinic name with no character limit.
Clinic URL field, allow visitor to paste the URL link to the field.
Visitor who submitted the submission name field, allow the visitor to type their
name.
Visitor who submitted the submission email field, allow the visitor to type their
email.
Submit button, allow visitor to submit the new clinic submission.
o New Member Registration Form
Username, allow visitor to create a username. Username name must have a
minimum of 8 characters and search through exiting database to ensure
username doesnt already exists.
Email, visitor must register a membership with a verified email address which
when registered, will required an email verification to verified their
membership. Only one email per membership is allowed. Should a visitor
register a new membership with an existing registered email, they will be
prompt, and given the option to log in with the registered email, unless they

forgotten their password, then will be prompt if visitor forget the password to
their old membership.
Password, allow visitor to select a password for their membership with an 8
character minimum. A prompt will appear to suggest visitor to strengthen their
password should it be needed.
Confirm password, allow visitor to confirm the password they had selected.
o Update Member Detail
First name, allow visitor to insert their first name. Visitors are ensured that their
private information such as their full name, date of birth and country of origin
will not be disclose publicly.
Last name, allow visitor to insert their last name. Visitors are ensured that their
private information such as their full name, date of birth and country of origin
will not be disclose publicly.
Date of birth, allow visitor to insert their date of birth. Visitors are ensured that
their private information such as their full name, date of birth and country of
origin will not be disclose publicly.
Gender, allow visitor to insert their gender (Male/Female). Visitors are ensured
that their private information such as their full name, date of birth and country
of origin will not be disclose publicly.
Country, the system will automatically display the country of origin of the visitor
who registers for a membership base on the cookies setting on their computer.
Visitor will not be able to change the country or origin.
DP upload, allow visitor to upload a DP to their member account.
Update button, allow visitor to update their account once all the additional
information has been inserted.

Change Request
Within the development period, a maximum of 3 (Three) change requests will be provided to the client.
Change Request will be referred to as the clients request to change, modify, adjust, and add elements in
the website while in the middle of the development and testing period. Additional change requests will
be quoted and charged accordingly.
Maintenance
In maintenance, we will be providing an optional annual Maintenance Retainer Service where we will
be obliged to do the maintenance for your website with a minimum turnaround time of 48 hours
depending on the availability of the developer and the level of service request.

The Maintenance Retainer Service scope is inclusive of the following:
A minimum of 48 hours turn-around time for maintenance service request.

Security patches for the website.
Website backup restoration in case the website encounters a problem.
If the client agrees to subscribe to the maintenance retainer service, separate additional fees would be
charged should the client opted for the services other than the maintenance retainer service's scope. The
charges are as follows:

To explain the table above:
If the client wishes to request for an Email Setup and fix a Domain Name & Web Host fix
issues within the month of January, they will be charged $100 for that month since the service
request is Bundle 1.
If the client wishes to do some Minor Design Changes within the same month, they will be
charged $160 instead of $100 since the minor web changes will be under Bundle 2.
All service maintenance done within the month will only be charged once within the same
month.
If the client wishes to do 2 or more service maintenance under the same bundle, they will only
be charged once within the same month and the charge will be the highest bundle.
If there are no maintenance service done on that month, there will be no charge within that
month.
Change Request will fall under Major We Changes if change requires more than 10 lines of
custom coding.
However, if the client does not wish to subscribe to the annual "Maintenance Retainer Service", it is
within CNAbiera Freelance Service's discretion to accept any maintenance service request.

Payment & Milestone

The total value for the services pursuant to this Statement of Work shall not exceed $2,700.00 (Two
Thousand Seven Hundred Dollars) unless otherwise agreed to by both parties via the project change
request procedure, as outlined within.

Project Agreement
The client listed under CONTACT DETAILS understands that this project will be handled by a freelance service and will keep all
information regarding this web project under strict confidentiality.
WEB PROJECT AGREEMENT

It is understood that this is a legally binding agreement between Carlo Nico S. Abiera, and the client listed under CONTACT
DETAILS.
It is understood that the document-signed Client Representative has the due authority to make legally binding agreements
with other companies and parties and has the authority to issue payments for the amounts detailed under PAYMENT &
MILESTONE.
Should the client and or the representative duly authorized by the client listed under CONTACT DETAILS wishes to terminate
this agreement after the SCOPE OF WORK is completed:
o No refunds will be given and the amount detailed above will still be due as an outstanding balance including
engaged maintenance service fees.
o The client listed under CONTACT DETAILS agrees that Carlo Nico S. Abiera holds the authority and ownership of all
files and information related to this web project including access details, images, text information and all other files
until the outstanding balance has been fully settled.
Should the client and or the representative duly authorized by the client listed under CONTACT DETAILS wishes to terminate
this agreement before the SCOPE OF WORK is completed:
o Considering that the client listed under CONTACT DETAILS has paid 50% (fifty percent) of the total amount of the
project and is within 4(four) months upon signing the statement of work, 40% (forty percent) of the total paid
amount will be refunded to client listed under CONTACT DETAILS irrespective of the site development stage.
o If the client listed under CONTACT DETAILS was not able to pay at least 50% (fifty percent) of the total amount of
the project, no refunds will be released by Carlo Nico S. Abiera.
o If the client listed under CONTACT DETAILS wishes to terminate this agreement after 4(four) months upon signing
the statement of work, no refunds will be released by Carlo Nico S. Abiera.
o The company listed above also agrees that Carlo Nico S. Abiera holds the discretion whether or not to release any
files or information related to the web project.
The web project will be considered completed if all of the listed Scope of Work has been fulfilled.
The client listed under CONTACT DETAILS agrees that 1 link will be added on the websites footer as a reference to the
developer, designer or SEO specialist.
MAINTENANCE AGREEMENT
It is understood that Carlo Nico S. Abiera will not be obliged to do any of service maintenance on the web project unless the
client listed under CONTACT DETAILS has purchased a Maintenance Retainer Service.
If the client listed under CONTACT DETAILS requests a service maintenance on the web project without purchasing a
Maintenance Retainer Service, Carlo Nico S. Abiera reserves the right to decline or accept the request.
If however, the client listed under CONTACT DETAILS subscribed / purchased the maintenance retainer service, Carlo Nico
S. Abiera will be obliged to perform the maintenance request and will have a turnaround response time of 48 hours
considering that all maintenance request work will be done after office hours.

CLINIC LISTING PAGE is a page where it displays the name of all the clinics in Korea. Clicking on the name of a particular
clinic will take visitor to the REVIEW DISPLAY PAGE for that particular CLINIC PROFILE that the visitor clicks through.
REVIEW DISPLAY PAGE will display all reviews submitted to that particular clinic. To access the CLINIC LISTING PAGE
visitors will have to select REVIEW through the PATIENT drop down from the MAIN LINKS. Clicking on any REVIEW
TEASER will take visitor to the FULL REVIEW PAGE

Clinic Listing Column Clinic Listing Colum will display under the CLINIC LISTING PAGE each individual Clinic Display
Column will take visitor to the REVIEW DISPLAY PAGE for that particular clinic visitor click on. For example, if a visitor
clicks on BK CLINIC profile display column, it will take visitor to BK clinic REVIEW DISPLAY PAGE where all reviews
submitted to BK Clinic are archived. Each individual Clinic Display Column will have the following display:
1) Name & corporate logo of the clinic. (Linkable to clinic profile panel)
2) Clinic Popularity Numeric Ranking (1st, 2nd, 3rd, etc.) Based on verified reviews archived under the clinic ONLY.

Dynamic Arrangement Dynamic Arrangement is when there is a new review submitted and archive under a particular
clinic, that particular clinic profile will display ahead of other clinic profile. Arrangement will be based on the date/time
the admin approved the review to be archive under a particular clinic, and not the date of the submission. For example, if

a visitor submit a review for BK clinic on 1st
October, and another visitor submit a another review for ABC clinic on the 2nd of October, if the admin approved the
review submitted on the 2nd October for ABC Clinic first, then approved the review submitted on the 1 st October for BK
Clinic, then the clinic profile for ABC Clinic will display ahead of BK Clinic. So arrangement of clinic profile will depend on
when admin approve a review to be archived. Any new clinic profile created by the admin will always be display at the
end of the CLINIC LISTING PAGE Dynamic Arrangement also applies to Clinic Profile Panel, Surgeon Profile Panel, and
Procedure Profile Panel, meaning the latest and most current review submitted and archive under that particular profile
panel will always display ahead of the old review.
REVIEW DISPLAY PAGE

Clinic Profile Panel, Surgeon Profile Panel, Procedure, Author/member Profile Panel
Clinic, Surgeon, Procedure, author/member Profile Panel profile panel display information about a particular clinic,
Surgeon, Procedure and author/memberlong with its archived reviews. Whenever a visitor click on a particular clinic
name, surgeon name, procedure name and author/member name, it will take the visitor to that particular profile panel.
The function work throughout the page wherever a clinic name, surgeon name or procedure name are displayed,
Meaning, a visitor could be viewing an eyelid surgery procedure profile panel and click away to view a nose job surgery
procedure profile panel. Similarly, a visitor could click away from nose job surgery procedure profile panel to view Dr John
surgeon profile panel. So the name of the clinic, surgeon, procedure and author/member will be linkable to the particular
profile panel.
Verified/Un-Verified Reviews Dropdown Display Dropdown display will provide visitor the option to display either
verified or un-verified, or all reviews archived under that clinic. By default, display will display combined reviews, both
Verified & Un-Verified reviews. Dropdown display will have the following dropdown option:
1) Combined by default, will display all reviews archived under the clinic, both verified and un-verified.
2) Verified will only display reviews archived under the clinic that is verified.
3) Un-Verified will only display reviews archived under the clinic that is un-verified.
Clinic Profile Panel Clinic Profile panel is basically the home page display of the clinic. It provides visitors with basic and
statistical information regarding the clinic base on the number of reviews archived under the clinic. Clinic profile panel will
have the following information:
1) Name & corporate logo of the clinic. (clinic name linkable to the clinic profile panel, corporate logo will enlarge when
click)
2) Last verified Review Submission date & time for the clinic. (1 day ago, 1 month ago, 1 year ago, etc.)
3) Clinic Popularity Numeric Ranking (1st, 2nd, 3rd, etc.). Based on verified reviews archived under the clinic ONLY.
4) Current Month Reviews submission, display the number of reviews verified review submitted and archived under the
clinic for the current month.

5) SUBMIT A TESTIMONIAL FOR SURGEON link Submit a testimonial for surgeon link allow visitor to leave a
personal testimonial for the surgeon should they choose to do so. This is an optional feature that visitor can choose
to or not provide testimonial for the surgeon. Submit a testimonial for surgeon link will have the following display:
a) A title field for the testimonial. There will be an 80 character limits.

b) A message field for visitor to write a testimonial for the surgeon. There will be no character limits.
c) A submit testimonial button to upload testimonial to the review.
6) Procedures you had done there will be an ADD button where if the visitor click on it, will allow user to select the
procedure via a drop down menu. User can click on ADD multiple times to insert multiple procedures to the review.

a) The first drop down field will allow visitor to select the category of the procedure.
b) The second field will allow visitor to select the procedure base on the category that the visitor has selected.

from the (Clinic) will suggest and display reviews from the same clinic. Refreshing the page will display a new review
suggestion from the same clinic. Other review from the (Clinic) will have the following display:
Other Related Reviews other related reviews will suggest and display review from other clinic or even the same clinic
base on the procedures listed in the review. For example, if the review the visitor is currently reading have eyelid surgery
listed as procedure the author/member had done, then the other related reviews will suggest and display other reviews
from other clinic or the same clinic that has eyelid surgery listed as the procedure the visitor had done. Refreshing the
page will display other related reviews from other clinic or the same clinic. Other related review will have the following
display:

New Clinic Submission Form Allow visitor to submit a new clinic submission that isnt already listed in the clinic listing.
A visitor doesnt have to sign or log in to submit a new clinic submission. New clinic submission will have the following
display:
1) Clinic name field, allow visitor to type the clinic name with no character limit.
2) Clinic URL field, allow visitor to paste the URL link to the field.
3) Visitor who submitted the submission name field, allow the visitor to type their name.
4) Visitor who submitted the submission email field, allow the visitor to type their email.
5) Submit button, allow visitor to submit the new clinic submission.
New Member Registration Form Allow visitor to sign up for a free membership on the website. once sign up, visitor
would have to verified their email with a verified link send to their email address. New member registration from will have
the following display field:
1) Username, allow visitor to create a username. Username name must have a minimum of 8 characters and search
through exiting database to ensure username doesnt already exists.
2) Email, visitor must register a membership with a verified email address which when registered, will required an email
verification to verified their membership. Only one email per membership is allowed. Should a visitor register a new
membership with an existing registered email, they will be prompt, and given the option to log in with the registered
email, unless they forgotten their password, then will be prompt if visitor forget the password to their old
membership.
3) Password, allow visitor to select a password for their membership with an 8 character minimum. A prompt will
appear to suggest visitor to strengthen their password should it be needed.
4) Confirm password, allow visitor to confirm the password they had selected.

Update Member Detail Once a member has
sign up for a membership, they will be prompt to complete their profile page by adding additional information about
themselves, such as a DP, their name, age, etc. Update member detail will have the following display field:
1) First name, allow visitor to insert their first name. Visitors are ensured that their private information such as their full
name, date of birth and country of origin will not be disclose publicly.
2) Last name, allow visitor to insert their last name. Visitors are ensured that their private information such as their full
name, date of birth and country of origin will not be disclose publicly.
3) Date of birth, allow visitor to insert their date of birth. Visitors are ensured that their private information such as their
full name, date of birth and country of origin will not be disclose publicly.
4) Gender, allow visitor to insert their gender (Male/Female). Visitors are ensured that their private information such as
their full name, date of birth and country of origin will not be disclose publicly.
5) Country, the system will automatically display the country of origin of the visitor who registers for a membership
base on the cookies setting on their computer. Visitor will not be able to change the country or origin.
6) DP upload, allow visitor to upload a DP to their member account.
7) Update button, allow visitor to update their account once all the additional information has been inserted.
