On your representation that:

You are free from any contagious diseases or other ailment and that you
are physically, psychologically and medically qualified (in accordance to the
supplementary questionnaire that you answered and signed which is hereby
attached and test that might be required to be submitted) for the job that you
have applied for; You warrant that you possess high values and character
required of a good citizen.

That you possess the proper educational/academic qualifications, work


experiences (if applicable) as enumerated below and relevant to your
assignment:

You likewise have or will submit the required clearances within an agreed
deadlines which you freely and willingly imposed on yourself whereby it
will be on your initiative . Your are aware and fully agree that your initial
day off on the first three to four months will be used solely for the following
stated bellow :

General note: It will be the employees sole responsibility to obtain, inquire, call,
fill up the required form, line up, advance money, take time out necessary for the
accomplishment of the following requirements. The employer will only provide the
necessary forms and guidance in attaining the purpose below.

A. NBI Clearance
B. Barangay Clearance
C. Police Clearance
D. NSO Birth Certificate or birth certificate
E. Medical Certificate.

The employer will not advance any payment for securing the above
requirements. After you have work for the employer for One year and Six
Months, the Employer shall reimburse you of all fees paid in securing the above

clearances/certificates. Proof of payment of expenses (receipts) should be


preserved for submission to Employer for reimbursement.

F. Voters ID or Postal ID for reference


G. Tax Identification Number.

From the above we are employing you on a probationary basis personal


family employee, with the designation of a family household staff. You are
prohibited from engaging in any other activities and/or businesses (sidelines).
You are expected to possess with all around knowledge in the house at home /or
designated home, under the following terms and conditions stated bellow:

1. The continued employment is based on the notion that you are fit
to work physically, psychologically(optional ) and possess all the qualifications
needed to discharge the duties of your position. You agree that you will get the
fit to work certificate and that you shall provide a waiver of the doctor patient
confidentiality with regard to your Medical Examinations, the findings and results
therefrom.

2. This probationary employment is for a period of ______ months,


effective _______________ , unless we notify you that your services will be
continued.

3. During the probationary period you will be working with us on a


trial basis and your conversion to regular status shall be determined on the basis
of such standards you are expected to be continuously evaluated, including, but
not limited to the following specific criteria/standards, to wit:

a. Quantity of work, job knowledge & skills, given the normal


training you are required to be able to carry out your job later on
unsupervised, calm, solid character, does not bring problem to the
house, reassuring, understanding, and composition;

b. Attitude & punctuality, interpersonal skills, initiative, judgment,


resourcefulness, utilization, dependability, leadership, planning, team
effort, adaptability and practicality;

c. Compliance with rules and regulations and other criteria or


standards that are reasonable, and duly disclosed beforehand by the
employer.

4. In the event your services as are terminated by the Employer


pursuant to law during this period, you will be entitled to collect only your wages
up to the end of working hours of the last day of actual service.

5. For giving your entire time and attention to the work assigned to
you during the working hours of this, you will receive a benefit package in the
total amount as indicated in the computation below:

a. Starting Gross Amount per Month P8,500.00 consisting of:


i. P200 per day salary for 2 days off worked every month .
Rest days not consumed will be converted to cash equal
to daily salary per day.
ii. P800 per week miscellaneous expenses allowance or a
total of P3,200 per month

a. The day off will be given provided that the household is in


order, all the given tasks and assignments are done and it will not
burden others.

b. Any day off not availed of in a month is understood and


agreed to be waived by you because the household is not in order,
not all the given tasks and assignments are done, further training
is needed, illness within the household, other unforeseen
circumstances, natural calamities or shortness of staff in the
house.

c. Any days off waived and worked shall be


correspondingly compensated.

d. The Rest days will sole be determined by the employer.

e. Rest days will be two (2) days per month, which the day
will be determined solely by the employer . This will increase if the
stability and efficiency of the house improved

f. The employee agrees to return to work after her Rest


day. Failure to return to work will result to verbal reprimand,
suspension or termination depending on the number of violations
made.

g. SSS, Pagibig and Philhealth

The employer shall pay the whole SSS contributions of the


employee.

The employee confirms that the employer provided as part of the


annex the SSS forms application. The employee is obliged to fill
up the SSS Form and process her registration with the SSS

The Employee agrees to release Employer from any liability in


relation with the Employees failure to apply and secure an SSS
membership number.

Upon presentation of the current SSS activated number which


you have to applyfor/verify on your own time, the contributions
thereupon will be for the account of the employer to pay .

h. To apply and pay for SSS, Pagibig and Philhealth

It is the sole and ultimate responsibility of the employee to go to the


SSS, Pagibig and Philhealth office with the Employers check
corresponding to the amount of contribution on your day off or
appropriate day with prior permission from the employer to
facilitate the physical payment thereof.

i. In no way that employee will attend to your SSS, Pagibig


and Philhealth matters whereby it will disturb her regular work schedule
or disrupt the orderly running of the household. However, the above
shall not be taken to mean that your deposit of the check for the
contribution as abovementioned with the appropriate collection agency
of the SSS, Pagibig and Philhealth may be made at anytime.

j. All checks for the contribution as abovementioned shall be


delivered/deposited/paid to the appropriate collection agency of the
SSS, Pagibig and Philhealth before the due date only allowing for delay
due to matters beyond the control of both parties, such as computer
breakdown, public holidays, due date on a weekend, force majeure and
other similar circumstances.

k. It is the Employees sole responsibility to fill up the form


accurately, present to the employer for him to issue the check within a
reasonable time,

l. to go to the SSS office and to provide the employer proof of


payment without the necessity of a demand.

m. ATM

Salary is payable on a monthly basis or on day off basis via a


bank account ATM cash card. It is agreed that all payments shall be
done through the ATM, cash card.

n. If for some reason Employee cannot open an ATM cash


card, then she will be given Three months commencing
______________ to attend to the opening of the account which shall be
at your own time and expense.

o. If after 3 months as agreed upon the Employee still fails to


open an ATM cash card account, the salary shall be directly paid to the
Employee on a cash basis.

p. The no cash inside the house policy shall be strictly


imposed at all times.

q. Employee confirms that she has been given a copy of the


application form for the Atm account to be submitted to the bank for
processing.

r. Pro rated 13th month pay shall also be given via the ATM
cash card.
5. Employee agrees to abide by all the rules and regulations of the
employer and village/homeowners/unit owners association, which it shall be
your duty to study and know immediately upon hiring. Employee confirms that
she has received and read her own personal copy of the said rules and
regulations given during the signing of this contract . You further agree that you
are aware that there is an administration office in the building that you can
approach at your own initiative and there is a house file freely accessible.
Should Employee commit any violation of the said rules which is penalized by
fine or any other penalty that requires payment or compensation, said payment,
including any interest or surcharges, shall be shouldered by the Employee and
deducted from her salary. Concealing a violation to the Employer is a grave
offense which shall cause dismissal from work. You are encourage to read, ask
the guards and staff what are the potential violations for you to prevent those
common mistakes.

6. Employee shall refrain during your employment with the employer,


from working for another employer whether within or outside your working hours
without prior written consent from the employer.

7. You agree to abide by all established working hours of your


employer.

8. The employer shall not be responsible for any expenses you may
incur in the execution of the duties of your position unless the employer, prior to
the incurrence of such expenses, duly authorizes you.

9. You agree that all records and documents of the employer and all
information pertaining to its business or affairs or that of any of its affiliated
companies are confidential, and that no unauthorized disclosure or reproduction
of the same will be made by you at any time during or after your employment with
us in perpetuity.

10. You shall be responsible for all employer materials or products


under your custody/control or those entrusted under your care and you hereby
agree to be liable for the cost or value thereof in case of loss or damage, such
cost or value you hereby authorize to be deducted from your salary.

11. You shall not reproduce, copy, download any copyrighted software
or program or other material/s owned by the employer that may come into your
possession in connection with your work or through any manner and means. Any
violation of the foregoing shall be a ground for outright termination.

12. You agree that all employer records, documents and assets in your
custody or control shall be immediately surrendered to the employer if requested
during the employment period, and at the termination thereof, whether or not
required.

13. Any misrepresentation made by you in your employment records


shall be a just cause for your dismissal.

14. You shall report any changes to the data you provided herewith,
your civil status and address for proper information and guidance.

15. You have the obligation to report to the employer any event, matter,
information or occurrence perpetuated that comes to your knowledge that is
violative of this agreement, the household rules and regulations and the peace
and order and well being of the employer, his heirs and assigns, and the
household. Any failure to abide by this provision shall make you liable as an
accomplice to the perpetrator.

16. In case of resignation, EMPLOYEE agrees to give ONE (1)


MONTH advance written notice personally given to and duly received by
EMPLOYER. Upon cessation of this agreement for whatever cause, the
EMPLOYEE shall voluntarily and peacefully turn over to EMPLOYER all
properties, IDs, records or documents, or other properties or assets belonging to
EMPLOYER or his family, that may be in his custody or possession without the
necessity of demand. Failure to comply on the part of EMPLOYEE shall allow
EMPLOYER to withhold issuance of his clearance, in addition to the filing of the
appropriate legal charges or action against him.

17. It is expressly agreed and understood that there are no verbal


agreements or understandings between you and the employer or any of its
representatives and agents affecting this agreement, and that no alterations or
variations of the terms hereof shall be binding upon either party to this agreement
unless the same are reduced in writing and signed by you and the employer.

18. Advances of salary are not allowed among each other or any staff
in the building. Likewise having loans among other staff is not encouraged and
any agreement between you and the other household employees shall be at your
own risk.

19. Taking pictures of the employers , his son , properties ,


workplace , relatives , friends and anything that is private in nature is not
allowed . In addition posting them in social media like facebook is not

allowed . The employer has the right to request the removal permanently of
any pictures of the said and the employee will have the pictures deleted .

20. Teaching my son ride on motorcycle or any activities that are


deemed by the employer to be dangerous or prohibited or his express wish for
the well being of the child as interpreted by the family is not allowed. Watching
violent films , scaring the child , screaming at the child , cruelty to animal ,
bringing the child inside the kitchen, unstructured playtime and leaving the child
unattended specially during park play times are not allowed

21. Having a relationship with person involved in the building or


neighbourhood is strictly not allowed . Allowing entry of unauthorized person in
the house will lead to termination. Sneaking out of the house without the prior
permission of the employer like visiting other units, common areas, leaving the
building is likewise a ground for termination

22. You are required to wear decent clothes when you are in the
common areas. You are required to wear your ID outside the unit at all time. You
are expected to return the id when your employment ends as it belongs to the
associations .

23. The employer has the right to have pictures which depicts the
Employer, his child or any part of his residential premises in Employees cell
phone removed.

I have read, understood and hereby agree to all the terms and conditions of
employment set forth in this contract, or have read them and explained to me in
the dialect I understand and I do hereby accept them.

My employer personally spent time with me together with another person to read
the contract, the supplementary interview, the foot notes, annexes, the rules and
regulation of the association and the household, which I duly signed as well as all
the attachments hereto.
