I, MACK DONALDS, of legal age, Filipino, single and a resident of

Brgy. Aglipay, Batac City, Ilocos Norte, after having been duly sworn to in
accordance with law, depose and state:

1. That I am employed as a messenger of Batac City Postal Office

located at National Highway, Quiling Norte, Batac City, Ilocos Norte;

2. That on January 10, 2017, I served a copy of the following letter-

request by personal service in accordance with Sec. 6 of Rule 13 of
the Rules of Court:

Psychological examination
Of Malyn Fernandez

in relation to Civil Case No. 1234-17 entitled "TYRONE DIEGO VS.

MALYN FERNANDEZ" by personally handing the letter to Malyn
Fernandez in her residence in Brgy. Caunayan, Batac City, Ilocos Norte
as evidenced by Registry Receipt No. 1111 attached herein and with
instructions to the post master to return the mail to sender after ten (10)
days if undelivered.
