Form No. 9), is the one utilized in the May 10, 2010 National and Local Elections. The ER page corresponding to local positions is found in page 5 thereof. Page 5 lacks the “DATA ON VOTERS anp/Resolution No. 9965 BALLOTS" and “INSTRUCTIONS”, normally found in page 1 of the ER. The BEls to manually write down the foregoing “DATA ON VOTERS AND BALLOTS” at the topmost portion of the ER; 2. The Election and Barangay Affairs Department (EBAD) to reproduce the “INSTRUCTIONS? for the filling up of the ER and the Administrative Services Department (ASD) to deliver the same to the CEO; and 3. The CEO to distribute a copy of the “INSTRUCTIONS” and this Resolution to the BEIs, during the conduct of the briefing on the General Instructions or at the time the BEls will get the EDCVL and PCVL. Let the Office of the Deputy Executive Director for Operations (ODEDO) implement this Resolution.
