CONTRACT OF SERVICE

TO ALL MEN BY THESE PRESENTS:

This Contract of Service made by and between the Department of Education, Region IV-MIMAROPA,
Oriental Mindoro a government agency duly represented by MUSTIOLA M. SAPALLO, Filipino of legal age,
resident of PINAMALAYAN, Oriental Mindoro, and employed as HEAD TEACHER III assigned at BULI
ELEMENTARY SCHOOL hereafter known as FIRST PARTY, and KEVIN PANGANIBAN, likewise
Filipino of legal age, with residence at, PINAMALAYAN Oriental Mindoro, hereafter known as the Contractor
and referred to as the SECOND PARTY, do hereby agree and state:

1. That the second party agreed to work for the LABOR for and in consideration of the sum of Php
1,500.00.

2. That the SECOND PARTY shall finish the job within ____ days after signing the contract
guaranty/notice to proceed.

3. That the SECOND PARTY shall be under the supervision of the FIRST PARTY.

4. That the payment for the service rendered the SECOND PARTY shall be given after the completion of
work, and

5. That the SECOND PARTY fail to finish the job within the period specified herein, the FIRST PARTY has
no obligation to add any payment for the labor but, but the SECOND PARTY to turn over the project
completion.
