JOINT-AFFIDAVIT OF LEGITIMATION

WE, YOSHITO WAKABAYASHI and NOVELIE P.

LAMOSA, both of legal ages, Japanese and Filipinos
respectively, husband and wife to each other and residents of
Mabini Extension, Digos City, Davao del Sur, Philippines, after
having been duly sworn to in accordance with law, do hereby
depose and say:

That we are the legitimate parents of the child named

KENTO LAMOSA who was born on September 17, 2012 at
Digos Maternity Lying-In, Magsaysay St., Digos City, Davao
del Sur;

That the above-named child was conceived and was

born by me, NOVELIE P. LAMOSA, at the period when I am
not yet married to my said sons father; Yoshito Wakabayashi;

That at the time of the conception of the said child,

there was no legal impediment for us to marry each other for
we were both single and legally capacitated;

That the fact of birth of the said child was registered at

the Local Civil Registrar of Digos City, Davao del Sur;

That we celebrated our marriage ceremony sometime

on February 14, 2013 at the 2nd Municipal Circuit Trial Court
Hagonoy, Davao del Sur and was solemnized by Hon. Rey
Andres;

That we are executing this affidavit in order to attest the

truth of the foregoing facts and to apprise the proper
authorities that we are the legitimate parents of the above
child for all legal purposes this may serve best;

Further affiants sayeth naught.
