INDIAN SEAMAN BOOK

To make it convenient for you to apply for an Indian Seaman Book, request you to keep
the below mentioned documents ready and then proceed to follow the step by step
instructions in the following pages.

1. You have to obtain a medical certificate from a DG approved medical


centre. Please inform the doctor that you need the certificate for applying
the Indian Seaman book.

2. 5 STCW Course Certificate details and INDOS Nos.


a) Elementary First Aid / Medical First Aid.
b) Personal Safety & Social Responsibilities.
c) Personal Survival Techniques / Proficiency in Survival Craft &
Rescue Boat.
d) Fire Prevention & Fire Fighting / Advance Fire Fighting.
e) Ship Security Officer (SSO) Course / Security Trng for Seafarers
with Designated Security Duties.

3. Passport.

4. RPSL Letter issued by Fleet Maritime Services (I) Pvt. Ltd. (If you do not
have this letter, please request for the same before applying or going online to
apply).

5. One Photograph of 3.5 cms. X 3.5 cms.

6. Credit or Debit Card or Internet Bank Login & password to make online payment.

7. Soft /Scan copies of 1 to 5 listed above.
