Dear Sir/Madam,

Good day to you!

In lieu of the above-mentioned subject, we would like to ask for your approval
for the transfer of our existing PLDT Telephone Line to a new location to give way for
the construction of a new building.
We have already approached your technical office here at Bayombong, however,
they advised us to directly inform your office and ask your permission for the said
relocation. Attached please find some pictures of the existing telephone line/cable.
