AFFIDAVIT OF LOSS
I, MILANY MAESTRADO MORALDE, of legal age, Filipino, and currently
residing at Quezon, Bukidnon, Philippines, after being sworn in accordance with
law, do hereby depose and say THAT:

1. I was issued by the Bank of Philippine Islands (BPI) IT Park Branch,

located at Apas, Cebu City, Philippines, one (1) BPI Automated Teller Machine
Card (ATM) Card.

2. On 13 December 2016 and to my dismay, my wallet was stolen and

inside said wallet was the aforesaid BPI ATM Card. Thereby, said BPI ATM Card
was lost and could not be found.

3. Despite earnest efforts to locate said BPI ATM Card, the same could no
longer be found.

4. I am executing this Affidavit to attest to the truthfulness of the

foregoing facts and for the reissuance of the abovementioned BPI ATM Card.
