SUMMARY

The Mega Chois Knitwear Ltd. factory is located in Savar, Dhaka in a rented building. Consisting building
which is six (G+5) storied presently. The factory occupies the 1st floor to 5th floor, and the ground floor is
occupied by Clean Wash Limited factory. The building construction started in 2001 and production started in
2009. The building has been formally approved for industrial purposes. The factory had approximately 511
workers, working on regular basis.

The Factory was surveyed for electrical safety by Woosun Energy and Construction Co., Ltd. (WEC). The
purpose of the survey was to identify significant electrical safety issues and to provide recommendations for
remediation based on applicable standards specified by the Accord.

The scope of this initial electrical safety inspection was limited to the review and identification of major
electrical safety issues. The inspection did not include identification of minor deficiencies, which would be
further dealt with as part of follow-up inspections.

The table below summarizes the major electrical safety issues identified during the inspection.
Recommendations have been provided to each finding.

The implementation schedule shall be developed by the factory to remediate each of the findings. The
specific timing of improvements, including any requested extensions due to design / installation constraints,
shall be submitted to the Accord for an approval.

FINDINGS AND RECOMMENDATIONS

FINDING NO: E- 1

CATEGORY: DESIGN, DRAWINGS & RECORDS

FINDING:
Electrical Single Line Diagram (SLD) is unavailable.

RECOMMENDATION:
Draw as-built electrical SLD mentioning all required information by qualified engineer and get it
reviewed by Accord.

FINDING:
Thermographic scanning of the entire electrical system has not been performed.

RECOMMENDATION:
Thermographic scanning of the entire electrical system must be performed twice in a year.

FINDING:
Lightning Protection System(LPS) has not installed yet
RECOMMENDATION:
Design and install Lightning Protection System (LPS) in the factory; the LPS designs must be
submitted to Accord before starting installation.

FINDING:
Cables supported in sanitary pipes partially
exposed.

RECOMMENDATION:
The PVC/rigid pipe used for surface wiring must
be continuous through-out its length and
properly supported (clamped with saddle, at
regular interval of 600 mm).

PRIORITY: P2 LT cables without proper layout, supported in


sanitary pipe.

FINDING:
Pole mounted transformer with oil leakage.

RECOMMENDATION:
Consult the REB/DESCO Engineer to inspect
the oil level of transformer. The cables from the
transformer must be supported by catenary
wires at both ends.

Cables from changeover switch to LT panel and

CATEGORY: SERVICE LINE

FINDING:
Cables in service are joints (butt joints) between
terminations.

RECOMMENDATION:
Existing joints must be checked and rejoined
with standard jointing kits (use PIB tape instead
of PVC tape). Joints must be supported and
protected against physical stress and damages.

PRIORITY: P2 LT cable from pole mounted transformer with


temporary jointing.
REMEDIATION TIMEFRAME: 1 WEEK

FINDING NO: E- 9

CATEGORY: SERVICE LINE

FINDING:
Factory's service cables have excessive
sagging/inadequate support.

RECOMMENDATION:
Excessive sagging should be fixed by adding
adequate support to avoid any arcing/flashing at
the Grid terminal/factory terminal.

PRIORITY: P2 Service cable (LT) are randomly supported


without protection.
REMEDIATION TIMEFRAME: 5 WEEKS

FINDING:
Smaller size cables connected to higher rating
control devices.

RECOMMENDATION:
MCCBs controlling circuits connected through
smaller size cables must be checked and
coordinated as per the connected load.

PRIORITY: P2 Wires connected haphazardly to higher rating


MCCB in production floor.
REMEDIATION TIMEFRAME: 2 WEEKS

CATEGORY: WIRING

FINDING:
Cables inside distribution board is disorganized.

RECOMMENDATION:
Organize all the power cables securely and
neatly inside distribution board to prevent from
electrical hazards.

PRIORITY: P2 Distribution panel inside the production floor.


REMEDIATION TIMEFRAME: 6 WEEKS

FINDING NO: E- 12

CATEGORY: BOILER ROOM

FINDING:
Wiring flexible PVC cables are used for wiring
around vertical boiler surfaces.

RECOMMENDATION:
Wires close/attached to boiler must be protected
from external heat. If it is unavoidable use
metallic flexible conduit, and power cables must
be kept at least 0.9m from hot areas.

PRIORITY: P2 Boiler cable termination to motor terminal not


properly done.
REMEDIATION TIMEFRAME: 4 WEEKS

CATEGORY: CABLE AND SUPPORT

FINDING:
Large exhaust fans in production floors are
directly controlled by the MCB.

RECOMMENDATION:
Large exhaust fans must be connected through
control device (DOL Starter) such that it will not
restart automatically when power is restored.

PRIORITY: P2 Large exhaust fan.


REMEDIATION TIMEFRAME: 3 WEEKS

CATEGORY: CABLE AND SUPPORT

FINDING:
Single MCB controlling lighting circuits.

RECOMMENDATION:
6 Amps MCB may be used to control at least 2
lighting circuits. Reinstall the MCB with required
numbers of lighting circuits.

PRIORITY: P2 MCB used production floor for several lighting


circuits.
REMEDIATION TIMEFRAME: 4 WEEKS

FINDING:
Busbar is very rusty.

RECOMMENDATION:
Clean the busbar and provide insulated coating.

PRIORITY: P2 Busbar inside the panel.

CATEGORY: SWITCHBOARD AND PANEL

FINDING:
Cables looped at the MCCB to extend power
supply.

RECOMMENDATION:
Extending main supply to SDBs from the MCCB
by looping must be avoided. The main panel
may be redesigned with required capacity
breakers and distribute branches to other sub-
panels.

PRIORITY: P2 Wires directly connected to MCCB terminal and


distributed to lighting circuit.
REMEDIATION TIMEFRAME: 2 WEEKS

CATEGORY: SWITCHBOARD AND PANEL

FINDING:
Panel not securely fixed to the foundation.

RECOMMENDATION:
Panel base must be securely fixed to the
foundation, with appropriate fastening devices.
Install base plate/top cover with cable gland
according to the respective cable size of cable
for entry and exit. Panel may be relocated
separately.

PRIORITY: P2 All the panels installed near the door (Entry/Exit).


REMEDIATION TIMEFRAME: 4 WEEKS

CATEGORY: CABLE AND SUPPORT

FINDING:
Cable from LT panel without support and
protection.

RECOMMENDATION:
Service cable must be properly supported and
protected on ladder/tray with metallic cover.
Cables transferring to respective floor must be
protected with protective accessories.

PRIORITY: P2
LT cables for the building without support and

FINDING:
LT and diesel generator installed near the drain.

RECOMMENDATION:
Remove and reinstall the LT panel in safer
location for operation. Construct brick wall for
generator to separate from open space with
proper ventilation and foot ways for operation
and inspection.

PRIORITY: P2
LT panel and diesel generator of factory installed
REMEDIATION TIMEFRAME: 6 WEEKS at the edge of drain.

CATEGORY: GENERATOR ROOM

FINDING:
Battery terminals are left open.

RECOMMENDATION:
Use insulated rubber cap to cover all the battery
terminals.

Diesel generator of the factory not being properly

CATEGORY: GENERATOR ROOM

FINDING:
Generator output terminal box without protective
cover.

RECOMMENDATION:
Install protective cover for the panel, and
baseplate for cable entry/exit with supportive
accessories. Cables may be supported in
ladder/tray with protective cover by removing
bricks and cleaning leaked oil.

PRIORITY: P2
