patient'scurrentminuteventilation(whetherhighornormal)isnotenoughtoallowadequateoxygendiffusion
intotheblood.Therefore,theAagradientessentiallydemonstratesahighrespiratoryeffort(lowarterialPCO2)
relativetotheachievedlevelofoxygenation(arterialPO2).AhighAagradientcouldindicateapatient
breathinghardtoachievenormaloxygenation,apatientbreathingnormallyandattaininglowoxygenation,ora
patientbreathinghardandstillfailingtoachievenormaloxygenation.

Iflackofoxygenationisproportionaltolowrespiratoryeffort,thentheAagradientisnotincreasedahealthy
personwhohypoventilateswouldhavehypoxia,butanormalAagradient.Atanextreme,highCO2levels
fromhypoventilationcanmaskanexistinghighAagradient.ThismathematicalartifactmakesAagradient
moreclinicallyusefulinthesettingofhyperventilation.
