AFFIDAVIT OF LOSS

I, JULIANA FELIPE-RUIZ, of legal age, married, Filipino

citizen and residing at Barangay Bonfal East, Bayombong, Nueva
Vizcaya, after having been duly sworn to in accordance with law, hereby
depose and state:

1. That I and my brother Rodolfo Felipe who is mentally ill are the
only children of Jose Felipe and Pasiona Inaldo;

2. That my father Jose Felipe and his father, Vicente Felipe, married
to Felipa Gallebo, are the registered co-owners of that certain
parcel of land identified as Lot 2169 of the Bayombong Cadastre,
containing an area of EIGHT THOUSAND TWO HUNDRED
SEVENTY SIX (8,276) SQUARE METERS, situated at Bonfal
East, Bayombong, Nueva Vizcaya and covered by TRANSFER
CERTIFICATE OF TITLE NO. T-66666, a copy of which is
hereto attached as Annex A;

3. That the Spouses Vicente Felipe and Felipa Gallebo died on

December 1, 1964 and January 21, 2002 in Bayombong, Nueva
Vizcaya. Machine copies of their respective Certificate of Facts of
Death and Death Certificate are hereto attached as Annexes B
and C;

4. That the said spouses have two children, namely: my father Jose
Felipe (married Pasiona Inaldo who and Teodora Felipe. Teodora
and her husband Alberto Acierda are both dead and left the
following as their surviving children: Leonardo, Carmen, Jackiely,
Eduardo, Gloria, and Alberto, all surnamed Felipe;

5. That because of old age, my father Jose Felipe has been

incapacitated for several years and since my only sibling is also
suffering from mental illness, I personally takes care of them and
in custody of all important documents of my parents such as the
owners duplicate copy of TRANSFER CERTIFICATE OF
TITLE No. T-66666;

6. That when the strong typhoon Yoyong hit the Province of

Nueva Vizcaya some five years ago, the Magat river overflowed
and flooded our barangay. The water almost reached the roofing of
our house and resulted to severe damage to properties and loss of
animals including some of my parents important documents
 which were all entrusted to me such as the owners duplicate copy
of TCT No. T-66666;

7. That I attest to the veracity of the foregoing statements and hereby

execute this affidavit for all legal intent and purposes.

IN WITNESS whereof, I have hereunto set my hand on this 5th

day of October 2016 at Bayombong, Nueva Vizcaya, at Bayombong,
Nueva Vizcaya.
