Apersonwithafamilyhistoryofkidneystonesmaybemorelikelytodevelopstones.Urinarytract
infections,kidneydisorderssuchascystickidneydiseases,andcertainmetabolicdisorderssuchas
hyperparathyroidismarealsolinkedtostoneformation.

Inaddition,morethan70percentofpeoplewithararehereditarydiseasecalledrenaltubularacidosis
developkidneystones.

Cystinuriaandhyperoxaluriaaretwootherrare,inheritedmetabolicdisordersthatoftencausekidneystones.
Incystinuria,toomuchoftheaminoacidcystine,whichdoesnotdissolveinurine,isvoided,leadingtothe
formationofstonesmadeofcystine.Inpatientswithhyperoxaluria,thebodyproducestoomuchoxalate,a
salt.Whentheurinecontainsmoreoxalatethancanbedissolved,thecrystalssettleoutandformstones.

Hypercalciuriaisinherited,anditmaybethecauseofstonesinmorethanhalfofpatients.Calciumis
absorbedfromfoodinexcessandislostintotheurine.Thishighlevelofcalciumintheurinecausescrystals
ofcalciumoxalateorcalciumphosphatetoforminthekidneysorelsewhereintheurinarytract.

Othercausesofkidneystonesarehyperuricosuria,whichisadisorderofuricacidmetabolismgoutexcess
intakeofvitaminDurinarytractinfectionsandblockageoftheurinarytract.Certaindiuretics,commonly
calledwaterpills,andcalciumbasedantacidsmayincreasetheriskofformingkidneystonesbyincreasing
theamountofcalciumintheurine.

Calciumoxalatestonesmayalsoforminpeoplewhohavechronicinflammationofthebowelorwhohave
hadanintestinalbypassoperation,orostomysurgery.Asmentionedearlier,struvitestonescanformin
peoplewhohavehadaurinarytractinfection.Peoplewhotaketheproteaseinhibitorindinavir,amedicine
usedtotreatHIVinfection,mayalsobeatincreasedriskofdevelopingkidneystones.

Whatarethesymptomsofkidneystones?

Kidneystonesoftendonotcauseanysymptoms.Usually,thefirstsymptomofakidneystoneisextreme
pain,whichbeginssuddenlywhenastonemovesintheurinarytractandblockstheflowofurine.Typically,
apersonfeelsasharp,crampingpaininthebackandsideintheareaofthekidneyorinthelowerabdomen.
Sometimesnauseaandvomitingoccur.Later,painmayspreadtothegroin.

Ifthestoneistoolargetopasseasily,paincontinuesasthemusclesinthewallofthenarrowuretertryto
squeezethestoneintothebladder.Asthestonemovesandthebodytriestopushitout,bloodmayappearin
theurine,makingtheurinepink.Asthestonemovesdowntheureter,closertothebladder,apersonmayfeel
theneedtourinatemoreoftenorfeelaburningsensationduringurination.

Iffeverandchillsaccompanyanyofthesesymptoms,aninfectionmaybepresent.Inthiscase,aperson

Sometimessilentstonesthosethatdonotcausesymptomsarefoundonxraystakenduringageneral

tests.Thedoctorwillalsoaskaboutthepatientsmedicalhistory,occupation,andeatinghabits.Ifastonehas
beenremoved,orifthepatienthaspassedastoneandsavedit,astoneanalysisbythelaboratorymayhelp
thedoctorinplanningtreatment.

Thedoctormayaskthepatienttocollecturinefor24hoursafterastonehaspassedorbeenremoved.Fora
24hoururinecollection,thepatientisgivenalargecontainer,whichistoberefrigeratedbetweentripstothe
bathroom.Thecollectionisusedtomeasureurinevolumeandlevelsofacidity,calcium,sodium,uricacid,
oxalate,citrate,andcreatinineaproductofmusclemetabolism.Thedoctorwillusethisinformationto
determinethecauseofthestone.Asecond24hoururinecollectionmaybeneededtodeterminewhetherthe
prescribedtreatmentisworking.

Howarekidneystonestreated?

Fortunately,surgeryisnotusuallynecessary.Mostkidneystonescanpassthroughtheurinarysystemwith
plentyofwater2to3quartsadaytohelpmovethestonealong.Often,thepatientcanstayhomeduring
thisprocess,drinkingfluidsandtakingpainmedicationasneeded.Thedoctorusuallyasksthepatientto
savethepassedstone(s)fortesting.Itcanbecaughtinacuporteastrainerusedonlyforthispurpose.

LifestyleChanges

Asimpleandmostimportantlifestylechangetopreventstonesistodrinkmoreliquidswaterisbest.
Someonewhotendstoformstonesshouldtrytodrinkenoughliquidsthroughoutthedaytoproduceatleast
2quartsofurineinevery24hourperiod.

Inthepast,peoplewhoformcalciumstonesweretoldtoavoiddairyproductsandotherfoodswithhigh
calciumcontent.Recentstudieshaveshownthatfoodshighincalcium,includingdairyproducts,mayhelp
preventcalciumstones.Takingcalciuminpillform,however,mayincreasetheriskofdevelopingstones.

PatientsmaybetoldtoavoidfoodwithaddedvitaminDandcertaintypesofantacidsthathaveacalcium
base.Someonewhohashighlyacidicurinemayneedtoeatlessmeat,fish,andpoultry.Thesefoodsincrease
theamountofacidintheurine.

Topreventcystinestones,apersonshoulddrinkenoughwatereachdaytodilutetheconcentrationofcystine
thatescapesintotheurine,whichmaybedifficult.Morethanagallonofwatermaybeneededevery24
hours,andathirdofthatmustbedrunkduringthenight.

MedicalTherapy

Adoctormayprescribecertainmedicationstohelppreventcalciumanduricacidstones.Thesemedicines
controltheamountofacidoralkaliintheurine,keyfactorsincrystalformation.Themedicineallopurinol
mayalsobeusefulinsomecasesofhyperuricosuria.

Doctorsusuallytrytocontrolhypercalciuria,andthuspreventcalciumstones,byprescribingcertain
diuretics,suchashydrochlorothiazide.Thesemedicinesdecreasetheamountofcalciumreleasedbythe
kidneysintotheurinebyfavoringcalciumretentioninbone.Theyworkbestwhensodiumintakeislow.

Rarely,patientswithhypercalciuriaaregiventhemedicinesodiumcellulosephosphate,whichbindscalcium
intheintestinesandpreventsitfromleakingintotheurine.

Ifcystinestonescannotbecontrolledbydrinkingmorefluids,adoctormayprescribemedicinessuchas
ThiolaandCuprimine,whichhelpreducetheamountofcystineintheurine.

Forstruvitestonesthathavebeentotallyremoved,thefirstlineofpreventionistokeeptheurinefreeof
bacteriathatcancauseinfection.Apatientsurinewillbetestedregularlytoensurenobacteriaarepresent.

Ifstruvitestonescannotberemoved,adoctormayprescribeamedicinecalledacetohydroxamicacid(AHA).
AHAisusedwithlongtermantibioticmedicinestopreventtheinfectionthatleadstostonegrowth.

opensurgeryandcanbeperformedinanoutpatientsetting.

ExtracorporealShockWaveLithotripsy
Extracorporealshockwavelithotripsy(ESWL)isthemostfrequentlyusedprocedureforthetreatmentof
kidneystones.InESWL,shockwavesthatarecreatedoutsidethebodytravelthroughtheskinandbody
tissuesuntiltheyhitthedenserstones.Thestonesbreakdownintosmallparticlesandareeasilypassed
throughtheurinarytractintheurine.

SeveraltypesofESWLdevicesexist.Mostdevicesuseeitherxraysorultrasoundtohelpthesurgeon
pinpointthestoneduringtreatment.FormosttypesofESWLprocedures,anesthesiaisneeded.

Inmanycases,ESWLmaybedoneonanoutpatientbasis.Recoverytimeisrelativelyshort,andmostpeople
canresumenormalactivitiesinafewdays.

ComplicationsmayoccurwithESWL.Somepatientshavebloodintheirurineforafewdaysaftertreatment.
Bruisingandminordiscomfortinthebackorabdomenfromtheshockwavescanoccur.Toreducetheriskof
complications,doctorsusuallytellpatientstoavoidtakingaspirinandothermedicinesthataffectblood
clottingforseveralweeksbeforetreatment.

Sometimes,theshatteredstoneparticlescauseminorblockageastheypassthroughtheurinarytractand
causediscomfort.Insomecases,thedoctorwillinsertasmalltubecalledastentthroughthebladderintothe
uretertohelpthefragmentspass.Sometimesthestoneisnotcompletelyshatteredwithonetreatment,and
additionaltreatmentsmaybeneeded.

Aswithanyinterventional,surgicalprocedure,potentialrisksandcomplicationsshouldbediscussedwith
thedoctorbeforemakingatreatmentdecision.

Extracorporealshockwavelithotripsy.

PercutaneousNephrolithotomy
Sometimesaprocedurecalledpercutaneousnephrolithotomyisrecommendedtoremoveastone.This
treatmentisoftenusedwhenthestoneisquitelargeorinalocationthatdoesnotalloweffectiveuseof
ESWL.

Inthisprocedure,thesurgeonmakesatinyincisioninthebackandcreatesatunneldirectlyintothekidney.
Usinganinstrumentcalledanephroscope,thesurgeonlocatesandremovesthestone.Forlargestones,some
typeofenergyprobeultrasonicorelectrohydraulicmaybeneededtobreakthestoneintosmallpieces.
Often,patientsstayinthehospitalforseveraldaysandmayhaveasmalltubecalledanephrostomytubeleft
inthekidneyduringthehealingprocess.

Oneadvantageofpercutaneousnephrolithotomyisthatthesurgeoncanremovesomeofthestonefragments
directlyinsteadofrelyingsolelyontheirnaturalpassagefromthekidney.

Percutaneousnephrolithotomy.

UreteroscopicStoneRemoval
AlthoughsomestonesintheureterscanbetreatedwithESWL,ureteroscopymaybeneededformidand
lowerureterstones.Noincisionismadeinthisprocedure.Instead,thesurgeonpassesasmallfiberoptic
instrumentcalledaureteroscopethroughtheurethraandbladderintotheureter.Thesurgeonthenlocatesthe
stoneandeitherremovesitwithacagelikedeviceorshattersitwithaspecialinstrumentthatproducesa
formofshockwave.Asmalltubeorstentmaybeleftintheureterforafewdaystohelpurineflow.Before
fiberopticsmadeureteroscopypossible,physiciansusedasimilarblindbasketextractionmethod.Butthis
techniqueisrarelyusednowbecauseofthehigherrisksofdamagetotheureters.
