AFFIDAVIT OF NO SALES/TRANSACTION
I, MELODY YURTCU, of legal age, Filipino, married and resident of C-
14, Centropolis Commercial Condominium, Villongco St., W.S.R. Sucat,
Muntinlupa, City, after being duly sworn to in accordance with law, hereby
depose and say that:

1. I am the owner of MYQ. TRADING, a single proprietorship duly

registered with the Department of Trade and Industry(DTI) with principal
office address at C-14, Centropolis Commercial Condominium, Villongco St.,
W.S.R. Sucat, Muntinlupa, City.

2. My said business did not commenced any operation for the year
2010, thus, there was no transaction for the reason that I was abroad in
that year and there was no other person who can take my palce and whom I
can trust to manage said business.

3. This affidavit is not intended to defraud creditors or any government

agency nor to evade taxes or legal processes, there being no such creditors,
government agency nor such legal process against the said firm.

4. I am executing this affidavit to attest to the truth of all of the

foregoing facts and for all its legal intents, effects, consequences and
purposes, including to satisfy the requirements of the Bureau of Internal
Revenue(BIR).
