AFFIDAVIT

I, CARREN JOY M. CORDERO, of legal age, single, Filipino and resident of

Block 6 Lot 6, Casimiro Town Homes, Phase 4, Pulanglupa, Las Pias City, after
being duly sworn to in accordance with law, hereby depose and say that:

1. From September 2008 to January 2011 I was employed as dental

assistant at Dr. Abdullah Al Mozher Dental Hospital and dental Polyclinic, Khamis
Mushayt, Al Hassa Branch, Kingdom of Saudi Arabia, and I was issued a certificate
of employment by my said employer.

2. From February 11, 2011 to August 15, 2011, I was employed as Staff
Nurse at Capiz emmanuel Hospital, Inc., located at Roxas Ave., Roxas City,
Philippines, and I was issued a certificate of employment as hereto attached.

3. I am executing this affidavit to attest to the truth of all of the foregoing

facts and for all its legal intents, effects, consequences and purposes, including to
secure an authenticated certificate from the Department of Foreign Affairs.
