AFFIDAVIT OF PERSONAL SERVICE

I, JANNETH M. LORENZO, of legal age, Filipino, single and resident of

Centro Poblacion, Culasi, Antique, after being duly sworn to in accordance
with law, hereby depose and say that:

1. On January 23, 2008, I was instructed by ATTY. PABLO U. DOMINGO,

JR., now counsel for the plaintiffs in Civil Case No. 49C for UNLAWFUL
DETAINER, MUNICIPAL CIRCUIT TRIAL COURT FOR CULASI-SEBASTE, Culasi,
Antique, to serve a copy of a DEMAND LETTER TO PAY AND TO VACATE of
same date and addressed to the BOARD OF DIRECTORS, THE PRESIDENT
AND GENERAL MANAGER and the OFFICER-IN-CHARGE, all of CULASI WATER
WATER DISTRICT, Centro Poblacion, Culasi, Antique.

2. On same date, I personally served a copy of said demand letter on

MR. SANTOS SALAZAR, Board Chairman of Culasi Water District at his store
located at Centro Poblacion, Culasi, Antique, and who personally received the
same demand letter by signing therein as having received.

3. Still on the same date at about 1:30 p.m., I proceeded to the office
of the Culasi Water District at Centro Poblacion, Culasi, Antique, in order to
serve one copy each of same demand letter on FERDINAND PERRY VALENTE,
the PRESIDENT AND GENERAL MANAGER, and on a certain lady informally
called LUNI, the known OFFICER-IN-CHARGE, but I was told that the former
had left the office and the latter informed me that she was no longer the
Officer-In-Charge, and, upon further instruction from said counsel, I served
two copies of said demand letter on GRACE LORENZO-LIM, - an employee
thereat there and then present, of legal age, of suitable discretion and
authorized to receive letters for the said WATER DISTRICT, who received two
copies of said demand letter, who signed therein as having received and
who, immediately after receiving the same demand letters placed them on
the office table of the said President and General Manager.

4. Said demand letter in the required number of copies has been duly
served on the Board Chairman, the President and General Manager and the
Officer-In-Charge, all of CULASI WATER DISTRICT.

5. I am executing this affidavit of personal service to attest to the

truth of all of the foregoing facts and for all its legal intents, effects,
consequences and purposes.
