AFFIDAVIT OF DISCREPANCY
I, JOMAR D. REGALADO, of legal age, Filipino, married and resident of
12 Irid, San Martin de Porres, Cubao, Quezon City, after having been sworn to in
accordance with law, hereby depose and say, - that:

1. My true and correct SURNAME is REGALADO, born on August 15,

1986 of parents Dedio Tabanera Regalado and Ligaya Silvestre David at
Kalookan City, Philippines.

However in my voters registration records issued by the Commission on

Elections, under VIN -1421-0335C-H1586JDR10000 my SURNAME erroneously
appears therein as REGAL,DO instead of my true and correct surname
REGALADO

Likewise, since at the time of issuance of the said voters registration

records I was still not married, my CIVIL STATUS that appears therein is
SINGLE. However, since I already got married to Noena V. Escano on April 15,
2014 as shown by my Certificate of Marriage under Registry No. 20149407, my
CIVIL STATUS that should now appear in my voters registration records is
MARRIED instead of single.

The foregoing discrepancy is merely incidental and very insignificant and

does not digress from the fact that the name JOMAR D. REGALADO and
JOMAR D. REGALDO refer to one and the same person, for all its legal
intents, effects, consequences and purposes.

This affidavit of discrepancy is not being done for the purpose of evading
creditors or avoiding prosecution in civil, criminal or administrative cases or
other legal process, nor to gain any advantage, there being no such creditor nor
such case/s nor such process against me.

I am executing this affidavit to attest to the truth of all the foregoing facts
and for all its legal intents, effects, consequences and purposes, including to
make the proper correction in my Comelec records as far as my correct surname
and marital status is concerned.
