JOINT AFFIDAVIT OF LEGITIMATION OF CHILD

We, Ferdinand De Vera and Teresita De Vera, both Filipinos, of legal

age, married and presently residing at #122 Lopez Jaena St., Aurora
Hill, Baguio City Philippines, having been duly sworn in accordance
with law, hereby depose and state:

1. That we have been legally married to each other on May 28,

1987 at the municipality of Agoo, La Union, a wedding
ceremony solemnized by Pastor Homer Bumanglag. A copy of
our marriage certificate is attached hereto and made an integral
part of this affidavit.

2. That prior to our marriage and outside of wedlock, a child,

named Cerafica De Vera, was conceived and born on April 9,
1995 at Agoo, La Union.

3. That at the time of the conception of said child, we were not

disqualified by any impediment to marry each other, and,
therefore, by virtue of our subsequent marriage, the said child is
now legitimated by operation of law, particularly Article 177 of
the Family Code of the Philippines (As amended by Republic
Act 9858 approved on December 20, 2009).

4. That we execute this affidavit to declare the truth of the

foregoing facts.

IN WITNESS WHEREOF, We have hereunto set our hands this 17 th

of January 2017, at the Marquez Law Office, 304 Porta Vaga Bldg.,
Baguio City, Philippines.

license number N25-10-005694 for Ferdinand De Vera and license
number N24-11-006788 for Teresita De Vera issued by the
Department of Transportation and Communication, Land
Transportation Office on December 14, 2016 and June 10, 2014
respectively at the Land Transportation Office in Baguio City, bearing
their photographs and signatures, known to me as the same person
who personally signed the foregoing instrument before me and
avowed under penalty of law to the whole truth of the contents of said
instrument.
