This package contains all the information needed to prepare
for Utah Swim Camps 2017. Please read through this package
carefully since it contains vital information for a successful camp
for the campers.
It is VERY IMPORTANT that you fill out the enclosed forms
and either send it in by mail to: University of Utah Athletics,
ATTN: Jonas Persson, 1825 E. South Campus Dr., Salt Lake
City, UT 84112, scan it and send it to Camp Director Michele
Lowry at mlowry@huntsman.utah.edu, or bring it to registration
on the first day of camp. Please understand that the camper is
NOT allowed to participate in the camp without all signed forms.

Information for Campers
Things to bring
As a part of this camp we will require you to bring certain things. Utah Swim
Camps are not responsible for lost or stolen belongings. We advise NOT to
bring valuable items and to label clothing and equipment to minimize the
risk of losing them. Please note that day campers should bring a daily pack
with all required items including spare clothing.

Registration
Registration will take part at the Ute Natatorium at time listed at the itinerary
(sent out 2-4 weeks prior to camp). At that time, overnight campers will
receive keys and keycard for the dorm. Staff, and parents volunteering, will
then move campers luggage from the pool to the dorm during the first water
session, 10-11am. Campers will check in after the first water session.

Information for overnight campers


Roommates
Campers will be staying two campers per room and are assigned by age and
roommate request. If you dont have a roommate request, you will be placed
in a room with someone near your age. Floors separate boys and girls rooms.

Information for day campers


Drop-off and Pick-up
It is VERY IMPORTANT that you inform assigned Coach immediately upon
arrival and departure. PARENTS - Please note this and inform your swimmer
of this repeatedly.

Parking
Free parking will be available in the Dumpke lot. Extended Day Campers that
drive themselves can park in that lot. Parents are encouraged to park in that
parking lot when they drop off or pick up campers. PARENTS: please come
and meet your campers at the pool when you pick them up, and
walk them to the pool when you drop them off. We are responsible for
the campers whenever they are on campus without their parents, and would
rather not see that they walk by themselves anywhere. In addition, if you
receive a parking ticket while parking in a parking lot other than the Dumpke
lot, we are not allowed to refund you. The Dumke lot is located north of the
pool and HPER complex (1729-1737 Student Life Way, Salt Lake City, UT

ASSUMPTION OF RISK

I, the undersigned, am the Participant named above. I am at least eighteen (18) years of age and
understand the terms and conditions in this Agreement.

I am familiar with the various activities that may be included in this Camp including, but not
limited to, instruction, conditioning, practices, games, meals, lodging and travel to and from
camp activities (Camp Activities). I understand that Camp Activities can include foreseeable
and unforeseeable risks including death, serious neck and spinal injuries, which may result in
complete or partial paralysis, brain damage, serious injury related to the eye and/or head, serious
injury to virtually all internal organs, serious injury to all bones, joints, ligaments, muscles,
tendons, and other part of the muscular/skeletal system, and serious injury or impairment to other
aspects of my body and general health and well-being. I further recognize that there are risks
associated with travel and that I could incur some or all of these injuries during travel to and
from Camp Activities. Knowing of these risks, I freely and voluntarily participate in the
Program and hereby agree to assume and accept any and all risk of injury or death.

I am also familiar with the rules of conduct and University policies relating to this Camp. I agree
to abide by the all of the operating procedures, including safety procedures outlined by the Camp
instructors, plus any directions given to me by an authorized University employee during the
course of the Camp.

WAIVER, RELEASE AND INDEMNIFICATION

I understand and acknowledge that the University of Utah ("University") is not an insurer of my
behavior, actions or participation in the Camp, and that the University assumes no liability
whatsoever for personal injuries or property damages to me or to third persons arising out of my
participation in the Camp activities. I hereby agree to release, waive, covenant not to sue,
indemnify and hold harmless the University, and all of their officers, employees and agents
(collectively the "Releasees") from any and all liability, claims, demands, actions and causes of
action whatsoever arising out of or related to any loss, damage, or injury, including death, that
may be sustained by me or loss or damage to any property belonging to me arising out of or
related to participation in the above named Camp, and excepting only such loss, damage or
injury as may be caused by the sole negligence of any Releasee.
I acknowledge that the University has strongly recommended to me that I seek medical advice
concerning my physical health, conditioning and abilities, prior to engaging in any Camp
Activities. I do not have any heart, respiratory or other health conditions that would prevent my
safe participation in the Camp Activities.

I have adequate health insurance to cover the costs of treatment in the event of any injury
(provide health insurance information below).

I hereby give my express consent in the event of injury for the University to obtain any
necessary emergency aid, anesthesia and / or operation, if in the opinion of the attending
physician, such treatment is necessary.

I agree that the site of any lawsuit arising out of or related to participation in the Camp shall be
Utah and that this Agreement will be governed by and construed in accordance with the laws of
the state of Utah, without application of any principles of choice of law.

I shall pay any attorney fees or costs incurred by the University in enforcing this Agreement.

If any portion of this Agreement is held to be invalid by a court of law, then it is agreed and
intended that all the remainder shall, notwithstanding, continue in full force and effect.

I HAVE CAREFULLY READ THESE TERMS AND FULLY UNDERSTAND THEIR


CONTENT AND AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A

RULES AND REGULATIONS
1. The use and/or possession of alcohol, drugs, tobacco, and any other illegal or inappropriate
substance and items (firearms, lighters, candles, matches, fireworks, incense, knives, squirt guns,
etc.) by campers is prohibited. Any items found will be confiscated and calls to parents and
appropriate law enforcement officials (if warranted) made. Violations are justification for
immediate expulsion from the Camp without refund. If it doesnt belong at camp dont bring it!
2. Campers are expected to conduct themselves as ladies and gentlemen at all times. Inappropriate
language and behavior will not be tolerated. Be respectful of the world class, collegiate
atmosphere at the University of Utah
3. Campers understand that bullying is a serious matter and will not be tolerated. Any camper found
to be bullying other campers will be expelled from the camp without refund.
4. ALL medications brought to camp must be noted on the appropriate camp forms.
5. Campers are expected to follow the rules and curfew hours set forth at the camp directors
meeting.
6. Portable stereos, game players, cell phones, etc. are not permitted outside of the campers dorm
room. Rooms must be locked at all times. Televisions, microwaves, laptop computers, plug-in
telephones, and other appliances are not allowed in the dorm. Expensive personal belongings,
jewelry, and excessive cash do not belong at camp. The camp assumes no liability for any lost or
stolen items.
7. In the Heritage Center, campers are not permitted in rooms occupied by people of the opposite
sex unless the door is wide open and under no circumstances after "lights out. In a suite, ALL
campers must remain in the living area ONLY. Violation will result in expulsion from camp
without refund.
8. Campers agree not to enter someone elses room without their permission and presence, or to
handle another campers property at any time. Campers may not switch rooms.
9. Visitors who are not part of swim camp are not allowed in dorm rooms. This includes parents.
Parents are allowed on camp floors only on check-in and check-out days, and may not take meals
with their camper in the cafeteria. Parents may visit with campers in the main lobby on the first
floor.
10. A curfew procedure will be explained at an orientation meeting on the first day of camp, and
enforced without exception. Absence from their room at bed check or the dorm without
supervision of a staff member is a serious matter and will be handled by the Director.
11. Loud and disruptive activity is discouraged. Quiet hours will be established and enforced.
Campers are allowed only on swim camp floors in dorm. All other floors, except common dorm
areas (i.e., first floor lobby, game room, cafeteria etc.) are off limits.
12. Campers and their parents are held responsible for any damage to rooms and furnishings.
Payment for such damage will be billed to the parents. In cases where a group incurs damage, the
person(s) directly involved along with each member of the group will be responsible for an equal
portion of the damage. Campers should report any problems to their counselors If the key is
permanently lost, a charge of $100 will be assessed for the lock change. If a swipe card is lost,
there is a charge of $10 for card replacement. and must be paid for prior to leaving camp.
Campers are expected to keep their rooms clean and in good order.
13. Campers are expected to dress sensibly and appropriately at all times.

14. Campers are expected to show courtesy toward fellow campers, recreational and club swimmers,
the Utah Swim Camp staff, the Utah Swim Camp coaches, and Dorm staff.
15. Campers must attend all camp sessions and be on time for roll call. If a camper requires medical
attention or help of any kind, they must contact a staff member immediately. Campers must walk
to and from the pool with their group and counselors, and are never to leave the pool or dorm
unless accompanied by a staff member.
16. Report any illness or injury to your counselors or group coaches. Campers missing more than
three pool sessions due to illness or injury will be sent home. Campers should be in good health
upon arrival at camp.
17. If a camper's behavior is consistently and severely at odds with the Camp's rules and regulations,
they will be sent home. Parents will be notified and required to make all necessary transportation
arrangements for immediate return home at their expense. There will be no refunds or credits
from the Camp. Multiple session campers will not receive a refund or credit of any camp fees for
sessions not attended if sent home early.
18. Have fun, work hard, and enjoy the world-class atmosphere at the University of Utah and Utah
Swim Camp! Your coaches, counselors, camp director and other staff members are available to
help you with anything. Please let a staff member know if we can help you.
