AFFIDAVIT OF IDENTITY
I, GERALDINE QUILES GIME, of legal age, Filipino, married and a resident of
Brgy. Managa-naga, Bulan, Sorsogon, after being sworn in accordance with law, do
hereby depose and state:
1. That I was born on January 21, 1971 at Brgy. Busac, Ligao, Albay to spouses
Macario Quiles and Corazon Dalma;
2. That I was married to Goodwill Gime on April 17, 1991 at Regional Trial Court
in Ligao City and we had five (5) children during our cohabitation;
3. That in all my public and scholastic documents and before I was married, I
have been using the name GERALDINE QUILES per instruction of my
grandparents who had been my guardian during my childhood;
4. That it was only recently when I obtained a copy of my certificate of Live Birth
with the National Statistics Office, when I came to know my name was
erroneously stated as GERALDINE QUELES instead of GERALDINE
QUILES;
5. That I hereby certify that my true and correct name is GERALDINE QUILES
and not GERALDINE QUELES;
6. That I hereby attest that GERALDINE QUILES and GERALDINE QUELES,
refers to one and the same person;
7. That the erroneous entry is due to inadvertence and honest mistake;
8. That I am executing this affidavit to attest to the foregoing facts;
