AFFIDAVIT OF LOSS
I, JOSELITO CELESTIAL PERA, JR., 31 years old, single, Filipino
citizen, and resident of Jacinto St., Pobalcion D, Camiling, Tarlac, Philippines,
under oath, do hereby depose and state THAT:
1. I am a holder of a Postal Identity Card, the number of which I can
no longer recall, issued by the Philippine Postal Corporation which I
usually keep in my wallet;
2. Recently, while I was organizing the contents of my wallet, I
discovered that the said postal identity card was missing so I looked
for it in my other wallets and other drawers inside our house but
nothing was found. Also, I asked my household members if they
have seen my the same but all answered in the negative;
3. All the diligent efforts exerted to locate my postal identity card
proved futile; hence, I now consider the same as lost and beyond
recovery;
4. My Postal Identity card was never pledged or used to secure any
obligation;
5. I am executing this Affidavit to attest to the truth of the foregoing
statements and for all legal intents and purposes it may serve.
