(COMES NOW PLAINTIFF)
I, Dyan Alba, of legal age and a resident of Quezon
City, represented by counsel hereby files a complaint for
ejectment, after having duly sworn in accordance with law,
depose and state:
1. That I, the plaintiff owns a house and lot at No.9
West Aguila, Green Corss subdivision, Quezon City;
2. That such property was leased to the defendant,
Glenda Miranda;
3. That the lease agreement had a term of two (2)
years starting December 1, 2014, at a monthly
rental of twenty five (25) thousand pesos
(P25,000.00);
4. That herein defendant defaulted in the payment of
rentals for six (6) months from January 1, 2015 to
June 30, 2015;
5. I, plaintiff, executed this document based on true
and personal knowledge of facts and the nature of
the action filed therein;
6. That no such action involving the same facts has
been filed in another court, quasi-judicial or
administrative tribunal;
7. That if such action has been filed in another court or
tribunal, I shall state the status of the proceedings
thereof in this Honorable Court.
PRAYER

It is therefor prayed by the plaintiff the following:


1. That the herein defendant be ejected from the leased
premises;
2. That the herein defendant to pay the defaulted
monthly rentals for six (6) months;
3. That she be ordered to pay damages and the
attorneys fees for the filing of the suit.
