AFFIDAVIT OF CONSENT
We, Sps. ROBERTO M. MANANSALA Jr. and JOANNA C. MANANSALA,
both of legal age, Filipino citizens, and residing at the City of San Fernando, Pampanga,
after having been duly sworn to according to law, hereby depose and state:
1. That we are the legitimate parents of Julienne Ricciana Calalang-Manansala,
a minor who is 8 yrs. of age;
2. That our sister, Rosanna Lillian M. Manansala, is going on a pleasure trip to
Singapore and Thailand from October 31, 2006 until November 3, 2006, and she is taking
our daughter, Julienne Ricciana Calalang-Manansala, with her for a vacation. Thus,
our daughter will be accompanied during this travel by our sister, Rosanna Lillian M.
Manansala;
3. That since our daughter is still a minor, we hereby give our consent freely and
voluntarily for her to travel abroad;
4. That in her trip, we are responsible for her round trip ticket fare from Manila to
Singapore and Thailand, and back, and we guarantee that she will not be a public charge
and will comply with all the laws and rules and regulations imposed upon transient
visitors in Singapore and Thailand;
5. That we are executing this affidavit in order to attest to the truth of the
foregoing circumstances, and for the purpose of giving our parental consent to our
daughters trip abroad, as well as of complying with the documentary requirements of
DSWD.
IN WITNESS WHEREOF, we have hereunto set our hand this 27th day of October
2006 at the City of San Fernando, Pampanga, Philippines.
