AFFIDAVIT OF LATE REGISTRATION

I, ANGELITO COJUTAN ALARCIO, of legal age, Filipino,
married and a resident of Rizal West, San Isidro, Isabela, under
oath, hereby depose and state THAT:
1. I am the biological son of SPOUSES APOLONIO ALARCIO
and MILAGROS COJUTAN-ALARCIO;
2. I was born on April 17, 1969 at Brgy. Rizal West, San
Isidro, Isabela;
3. The fact of my birth was not registered in the Local Civil
Registry of San Isidro, Isabela due to inadvertence. As
proof of the fact of my birth, I am attaching herewith the
Joint Affidavit of Two Disinterested Person executed by
Zenaida Baniqued Villanueva and Teodora Solis Baniqued
which is hereto attached as Annex A;
4. I am executing this affidavit to attest to the fact of my
birth on April 17, 1969 at Brgy. West, San Isidro, Isabela
for the purpose of the late registration of my birth.
IN WITNESS WHEREOF, I have hereunto affixed my
