Greetings!
We, the 3rd year students of BS HNCA would like to ask for your permission to
conduct a survey and BMI assessment to the faculty of College of Arts and Sciences
as a part of our project in Sports Nutrition subject. The survey form will tackle about
stress management and will be focusing on a wellness program project entitle of:
Managing the stress of CAS faculty.
The objectives of this wellness program are:

To determine the rate of stress of the teachers.

To determine factors that causes their stress.
Effects of stress on workplace and health.
To formulate / conduct activities that would ease their stress.

We hope to make a positive impact to the faculty of CAS department of

Manila Tytana Colleges through the help of this project.
