HAZARD IDENTIFICATION PLAN


1.0 - INTRODUCTION:This Emergency Response Plan [ERP] has been formulated for implementation in
accordance with applicable Construction Safety Manual, Contractor Safety and
Administrative Requirements, safety polices and standards. This is a confirmation of
the commitment that X company shall safeguard; protect lives, and property of
Saudi Aramco and any other third party in connection with the work therewith to be
performed.
1.1 COMPANY POLICY:X maintains a Quality Management System to ensure that its products and services
are of a consistent high quality to satisfy our customers' requirements and thus
ensure our own and our client's long-term effectiveness and success. In addition, all
our systems are focused towards meeting the requirements of national and
international standards and environmental requirements.
We are committed to complying with these requirements and continually assess and,
where necessary, improve the effectiveness of our Quality Management System
through customer feedback and periodic management reviews.

1.2 PURPOSE OF EMERGENCY RESPONSE PLAN:The objective of this Emergency Response Plan is to ensure that complete Safety is
maintained at the job site and also therewith ensure safety of the property of Saudi
Aramco as well.
All personnel shall be familiar with facility emergency plan.
Designated an Assembly area in safe location to be used during emergencies.
Provide a list of contact numbers from the Contractor personnel and Aramco
personnel that are essential to call during an emergency including names,
position, responsibility and position.
Supervisor should be responsible for head count.
Familiar to the location where resources are available.
Personnel shall be familiar to audio and visual alarms of the facility.

Evacuation routes and procedures must be available.

2.0 SCOPE OF WORK:The scope of work describes the job required for the Replacement of old
PACUs and Installing of New PACUs with its ducting works ,Electrical works
and Testing.
Contarctor shall disconnect the power and associated lines for removing
PACUs .
Removing of existing duct and its supports to facilitate for Installing new
PACUs.
Contractor shall remove the existing PACUs from the platform.

Contractor shall mobilize the removed PACUs from work site to

Reclamation site.

Excavation for extending the existing foundation to fit for new PACUs..
Fixing and connect rebars from the existing foundation to get more
strengthen for foundation.
Form work and fixing of plywood and its wood supports for foundation
Pouring of 4000 PSI concrete in the respective prepared areas and to be
concreted.Epoxy Painting to be apply around the foundation with two
coats.
Curing of concrete with proper Interval of time as per SA standards.
Removing of form work and do repair works in foundations.
Installation of New PACUs in the respective foundation and provide
vibration Isolator springs on concrete base.
Provide and install flexible duct connector to unit and duct.
Fabricate and Install ducting connections from new machine to existing
supply and return ducts.

Supply and install two Inch fiberglass insulation with vapor barrier for duct
connections only.
Fabricate and install aluminium cladding on new duct connection.
Provide and install power cable from unit to the disconnect switch.
Provide and install duct on angle support.
Provide and install sand trap louver.
Weather proof the exterior duct with foster to prevent damage to
insulation.
Install two DDC panels ,one for each yard serving two units on provided
base.
Provide and install indoor rigid conduit for with fittings
Provide and install PVC conduit on exterior of building only with fittings.
Provide and install control wire.
Provide and install variable frequency controller.

Provide and install over load for each compressor.

Provide and install field devices.


Install power connections and hook up to DDC control panel from the
provided circuit breakers.

3. SCOPE vs WORK PERMIT SYSTEM


1. All the scaffolding activities for Installing Duct,supports and laying
conduits ,control cables from roof are Involved and all works will be
perform in cold work permit system. A proper full body harness will
be provided for protection and a standby man will be assigned for
ensure the work safely.
2. Most part of the Duct work and angle support which includes,
fabrication & cladding to be done in contractors yard and in which all
the works activities will be carried out in hot work permit system with
appropriate firefighting equipments like UL rated fire extinguishers,
fire blanket, fire water line hose & H2S detecting sensor.
3. Installation of PACUs by lifting crane with SA certified operator and
rigger and in which all the works activities will be carried out in hot
work permit system with proper method of statement procedure and
appropriate

4. For isolation and de-energization of the electrical the hot work permit
will be taken and proper lockout & hold tag will be installed at main

maintenance department.
5. Removing concrete and breaking will be done with hot and cold work
permit in which proper equipment will be used for demolishing with
ensuring all the equipment will be furnished with dead man switch.

6. For Testing and commissioning of PACUs units and associated devices


will be carried out with in hot work permit system prior with coordination and approval from operation and maintanence dept.
7. All the tools, material & equipment will be loaded in any one of the vehicle
like pick up, dump truck, boom truck and trailer in which all the vehicle will
be driven by a valid Saudi license driver and all the items will be transfer to
Juaymah warehouse unit.

6. CREDIBLE EMERGENCY SCENARIOS:-

This ERP is based on the identification of principal hazards such as fire,


explosion, and hazardous material release. These hazards and their associated
scenarios are assessed to determine both probability of occurrence and
consequences by the Unit OME. Any hazard identified and assessed as
significant is reviewed carefully to see whether it can be prevented or
mitigated by engineering, administrative controls, or Personnel Protective
Equipment (PPE).

time of any emergency.

Informal Hazard Review


The Unit OME performs an informal hazard review. The Unit Engineer
consults with Fire Protection, Loss Prevention and Compliance and safety
group on risk management and emergency response. The Unit OME identifies
potential internal and external emergency situations and reviews past incidents
to determine if they are still likely events. Incidents at similar facilities are
also reviewed. Interviews with operators, maintenance, Shift Superintendents,
and inspectors of the facility help identify less obvious problems and potential
incidents. They also perform an analysis that tracks the hazard from its source
to potential areas of damage to determine how people, property, or the
environment is affected. The team reviews credible causes and events
including such things as loss of containment, incompatibility of chemicals,
process condition upsets, equipment malfunctions, etc.

7.OTHERS
Health and Environmental Monitoring
Health and environmental monitoring shall be conducted by the contractor, as
applicable ( e.g., as identified in the HIP), to protect their personnel
Again exposure to health hazards (e.g., radiation, H2S, respiratory, noise).
Monitoring shall be performed by qualified personnel and the results shall be
submitted to the SAPO for review upon request.
Contract Supervisor should educate his co-workers prior to work start
regarding the location of assembly point area and the breathing apparatus
during the emergency period and also should locate the first aid kit box in case
of minor injury.
FIRST AID
contractor shall provide first aid facilities on all sites for his employees. The
first aid facility shall be kept in a sanitary condition all times. Minimum
requirements shall include the following : hot and cold water; wash basin; air
conditioning; adequate lighting; and dust tight medical supply cabinet.
First aid supplies shall be kept readily available in a cabinet designated for
those supplies only.
This cabinet shall be placed under the charge of first aid attendant, who shall
ensure the cabinet is well stocked at all times.
The cabinet content of the a first aid includes the following items Gauze pads
(at least 4 x 4 inches), Box adhesive bandages (band-age),one pack gauze
roller bandage at least 2 inches wide , Two triangular bandages, wound
cleaning agent such as sealed moistened to welters, Scissors, At least one
Blanket, Tweezers, Adhesive tape, Latex gloves, Resuscitation equipment
such as resuscitation bag, airway, pocket mask, Two elastic wraps, Splint &
Directions for requesting emergency assistance.

Emergency Training

All the personnel involved in the job activities shall be properly educated and
trained on all hazards which includes H2S, fire accident, Electrical shocks and
physical exercise .

Also giving prior information to all the personnel involved in the job
activities regarding the location of fire equipment and emergency exits during
occurrence of any potential hazards associated with operations.

If any hazards occur all the personnel have to be assemble in assembly area
during emergency period.
Heat Stress
Provide drinking water stations (e.g., coolers with chilled or ice water ) for
workers and remind them to drink plenty of water even if not thirsty.
Replenish drinking water supplies as needed to ensure availability and provide
more frequent collection of sanitary waste water from portable toilets.
Monitory the related condition of employees, as well as the effectiveness of
any engineering/administrative controls and personal protective equipment
(PPE) being used.
Providing measures such as shading and local ventilation when work in direct
sunlight is required.
Scheduling work/rest rotations for workers according to the current heat
index, which is based on temperature and relative humidity?
Conduct periodic ( e.g., weekly) safety meetings/talks during hot weather
emphasizing heat stress hazards and precautions.

8.IMPLEMENTATION OF WORK PERMIT


Procedure in implementing work permit system shall comply with G.I 2.100
of Saudi Aramco.
Works permit Issuer (Saudi Aramco Representative) and Receiver (Contractor
Representative)
The work permit Receiver of the contractor shall inspect jointly with the work
permit issuer who is Saudi Aramco representative. Using prescribed work
permit form, required information filled up in spaces provided in the form and
according to the type of work permit: Hot, Cold, Confine space entry and
Release Permits. There are items to be ticked for additional information
actually applied in the construction site. Additional precautions maybe added
to ensure safety measures are integrated in the implementation of work
activities as shown in the work permit. The work permit is signed both by the
work permit Issuer and Receiver to manifest their agreement on the
procedures adopted and conditions of the work site. It shall be considered
open to mean the stipulations in the work permit are enforceable at specified
time and duration.
For more than one day of permit period, the proponent superintendent will
countersign on the work permit to approve the length of time requested
Work permits will be jointly closed by the work permit Issuer and work
permit Receiver upon its duration and kept on the respective records of files
for safe keeping. A new work permit will be applied for the remaining part of
the construction work activities, same procedures will be followed as above.

General timing for working in the Juaymah warehouse will be 8 to 10


hours per day.
Normally hot and cold work permit will be taken for this entire job and
no requirement and need of taking confined space and entry release
permit.

chemicals or hazardous materials like paint & cements.
Heavy equipment operators shall have valid Saudi arab government
drivers license and also certified by Saudi Aramco.
A proponent shall evaluate if the management change process (MOC)
shall apply for any modification included in the project and proceed
accordingly with the processing.

9. EMERGENCY PREPAREDNESS
Project personnel shall also be oriented with the available telephone facility
nearest the workplaces for readiness of emergency response.
Fire drill exercises shall be conducted in the area to allow employees to
familiarize the area when emergency situation arises in the future.
In tool box meetings, contractor employees in worksites shall be taught with
proper emergency response using the Saudi Aramco standard procedure for
calling 110.
Text below shall be posted in conspicuous places to guide employees during
emergencies.
Emergencies can create a variety of hazards for workers in the impacted area.
Pre paring before an emergency incident plays a vital role in ensuring that
employers and workers have the necessary equipment, know where to go, and
know how to keep themselves safe when an emergency occurs.

9.ACTIONS TO BE TAKEN DURING EMERGENCY


Upon hearing an alarm all personnel shall stop all work immediately and
cancel work permit.
Shutdown all equipments safely.
Evacuate to the designated Assembly area.
Supervisor will conduct the head count,remain calm.

Remain at assembly area until all clear alarm


Revalidate all work permits prior to start the work.
10. EMERGENCY REPORTING
All emergencies shall be reported immediately (e.g., to the nearest SA
Security Main Gate or Security Control Center [SCC]).
Signs shall be conspicuously posted at the work site with the emergency
telephone numbers and reporting instructions.
Reporting emergencies within Saudi Aramco shall be:
A. By telephone. Dial 110.
B. By radio. Radio an operations group that has a telephone and ask
them to dial 110 if you cannot contact the Security Main Gate or
SCC directly.

Below is the standard text of Saudi Aramco emergency response procedure


which shall be posted conspicuously in the construction sites.

Responding To An Emergency
A Checklist For Rescuing A Suspended Worker
When an emergency occurs
If a suspended worker cant perform a self-rescue, call the on-site
emergency-response
team and get the appropriate rescue equipment.
First responders should clear a path to the victim. Others should direct
emergency personnel to the scene.
Prohibit all nonessential personnel from the rescue scene.
Talk to the worker and try to determine the workers condition.

If the worker is accessible, provide comfort and check vital signs. If


necessary, administer CPR and attempt to stop bleeding.
If the workers injuries are minor, proceed with the rescue. Only trained
responders shouldattempt a technical rescue.
If the worker has severe injuries, contact emergency medical responders.
Remember, 110 responders may not be able to accomplish prompt rescues.
After Any Emergency
Report fatalities and catastrophes to SA with in Eight hours. Report
injuries requiring overnight hospitalization and medical treatment other
than first aid with in 24 hours
Identify all equipment that may have contributed to the emergency. Let
a qualified person one who has the knowledge, the experience, and the
demonstrated ability to resolve fall protection and rescue problems
examine the equipment. If the equipment was damaged, repair or
replace it. If the equipment was damaged , repair or replace it. If the
equipment contributed to the emergency.
Determine what caused the emergency. Be as specific as possible.
Include dates, times, environmental conditions, work processes, and
persons involved
Review the emergency-response plan. Determine what procedures
should be added or changed to prevent similar emergencies. Revise the
plan accordingly.
