AFFIDAVIT OF NO EARNINGS
I, ESTER M. MENDOZA, of legal age, Filipino, single and
resident of #23 Ligaya Pascual St., BF Resort Village, Talon 2, Las
Pias City, after being duly sworn to in accordance with law, hereby
depose and say that:
I have never been employed all my life with any company.
I am a bonafide member of Social Security System under SSS
No. 33-4017569-2.
I became a member of SSS and registered as an SSS self
employed member, when I managed to start a small business as
sidewalk Vendor, and paid my SSS monthly contributions as selfemployed from April 2001 to March 2014.
I am not gainfully employed, with no source of income, and
wholly dependent upon my relatives for my chief support.
I have paid at least
or more than 120 SSS monthly
contributions, thus entitled to SSS monthly pensions.
I am executing this affidavit to attest to the truth of all of the
foregoing facts and for all its legal intents, effects, consequences
and purposes, including to satisfy the requirements of the SSS to
facilitate collection of my monthly pension.
