I, SUSANA S. SARAO, of legal age, Filipino, married and resident of #116
Tanawan St., Ayala Alabang Village, Muntinlupa City, after having been duly
sworn to in accordance with law, hereby depose and say that:

I am a holder of original building permit for the property situated at

#15 Villa Cristina Ave., Pamplona 3, Las Pinas City,
issued by the
Engineering Department of Las Pinas City.
The said original building permit was inadvertently lost.
Despite of diligent efforts to look for said original building permit, all
such efforts proved futile and in vain, thus, I am declaring the loss of original
building permit.

I am executing this affidavit to attest to the truth of the foregoing facts

for all its legal intents, effects, consequences and purposes.
