AFFIDAVIT OF DISCREPANCY
I, DARCY CALAMBA ANTONIO, of legal age, Filipino, and resident of Block 6
Lot 11, Gen. Malvar St., Almanza Uno, Las Pinas City, after being duly sworn to
in accordance with law, hereby depose and say, - that:
My true and correct name is DARCY CALAMBA ANTONIO, born on September
5, 1970 at Matiao, mati, Davao Oriental of parents Rosalinda Calamba and
Godofredo Antonio as appearing in my certificate of live birth issued by the National
Statistics Office (NSO).
However, in my certificate of baptism issued by San Nicolas

De Tolentino

Cathedral, my name is recorded as DARIO CALAMBA ANTONIO instead of my true

and correct name DARCY CALAMBA ANTONIO.
The foregoing discrepancy in my name is merely incidental and very
insignificant and does not digress from the fact that the name DARCY CALAMBA
ANTONIO and DARIO CALAMBA ANTONIO refer to one and the same person for all
its legal intents, effects, consequences and purposes.
I am executing this affidavit to attest to the truth of all the foregoing facts
and for all its legal intents, effects, consequences and purposes, including to make
the proper corrections of my name in my certificate of baptism from DARIO
CALAMBA ANTONIO to DARCY CALAMBA ANTONIO, and to request the said parish
church the issuance of a new certificate of baptism with the correct name.
