I, ROWENA PARUNGAO, of legal age, Filipino, widow, and resident of Lot 15
Block 8, Brown St., Veraville Classic, Vista Grande, BF Resort Village, Las Pinas City,
City, after being duly sworn in accordance with law, hereby depose and say, that:
I am the legal and surviving spouse of ANDREW BASILIO GUISON, who died
intestate on July 26, 2009 at Roasrio, Pasig City.
He was a holder of the following identification cards and other cards:

On or about September 26, 2009, during the typhoon Ondoy at above-stated

residence all the aforesaid identification cards and other documents belong to the
deceased were all washed out from flood waters.
Inspite of diligent efforts to look for all the aforesaid cards all such efforts
proved futile and in vain.
I am executing this affidavit to attest to the truth of the foregoing facts and
for all its legal intents, effects, consequences and purposes.

On March 7, 2014 at about 7:00 P.M., I parked the said vehicle along Abel
Nosce St., cor. Aventine St., BF Resort Village, in front of Avy Bahay Kubo
Restaurant, and left the said vehicle to take our dinner together with my sister and
daughter at the said restaurant, and upon my/our return to my said vehicle at
about 7:45, I was surprised to know that the left side mirror was broken and
discovered that my bag (Mendrez Bag) containing all the aforesaid items was lost.
3. Inspite of diligent efforts to look for said bag and all the aforesaid items
all such efforts proved futile and in vain up to this time, thus, I am now declaring
the loss of my bag and all the aforesaid items.
4. I am executing this affidavit to attest to the truth of the foregoing facts
and for all its legal intents, effects, consequences and purposes, including for the
issuance of another cards in lieu of the ones that were lost.
