Maripajo Street, Meadowood Executive Village, Bacoor, Cavite after having been
subscribed under the Law, do hereby depose and state that:
1. On 7th March 2001, when I came to the office of the ECE REALTY & DEVT.,
INC., I found out that my Contract to Sell for Fantasy World Theme Parks, Amusement and
Recreation Club, Inc. FW# 4768 was given to my agent.
2. However, the said Contract to Sell was never delivered to me by my agent.
3. I am executing this affidavit to attest to the truth of the foregoing and for the
purpose of securing a certified xerox copy of my Contract To Sell.
