1.General Rule:
* Professional Cook and Eatery Operators are not allowed to participate
* Each contestant must be in proper cooking attire
* Observe safety measures during contest duration
* The entry dish should represent filipino style.
* No duplication of menu.
* Each contestant must submit personal data on or before December
08,2014.Application Form is available at MSWD Office
* Must be a member of an accredited/registered organization within Siay.

11. Rules to follow by the Contestants:


Each participant will produce the following;
* Food or Dessert items to be cooked including all ingredients.
* Suggested Quantity: a minimum of 1 Kilo fish and 2 kilos Calabasa.
* Each participant will select only one category (Pili lang sa sud-an or sa dessert.
* Each participant will bring her cooking equipments.

111. Role of the LGU:


*Provide a cooking space, in front of the Municipal Hall.
*Provide water to be used before during and after cooking.
*Provide Prizes to winners

1. Feedback of the Pre-Assessment regarding the functionality of the Local Social


Welfare and
Development Officer
MSWDO, Ms. Marina T. Bayawa give feedback regarding the Pre-assessment of the
functionality of the LSWDO that was happened on October2, 2014. She stressed out that the
mission and vision and goal of the Office must have a resolution from the SB and each staff
needs to memorize. Likewise, she explained that that the functional organizational and
management structure of which it needs a file on one folder certain documents like the PDS,
Job description and appointment. O.C Fabillar was assigned to collect from the staff. MT
Bayawa, reminded everyone to make a (IPCR) Individual Performance Commitment Report
from the OPCR.-the whole performance commitment report which the MSWDO already make
and submitted.
She also explained that when there is a disaster the staff must make a documents and
acknowledgement receipts from the in and out of the goods.
Each one need to log-out in the logbook the trainings she/he attended. She also
reminded that file of the
payroll must in one folder.

Note: Secure list of Volunteers from the council of the Greeks, Women and DCW.

record/file of their individual program.

V. Rationale:
In consonance with the Pantawid Pamilyang Pilipino Program (4Ps), the National
Government aims to develop childrens potentials through Health and Education aspect. In this
way, they will be capacitated to contribute to nation- building as they grow up. Relevant to this,
there was a national designed activity for children who are 4Ps beneficiaries age 10-14 years
old known as; The search for 4Ps Exemplary Child from Municipal, Provl., Regional up to
National level. This is intended for 4Ps children who excel in their school performance.
V111. Objectives;
-To expose and recognize 4 Ps Childrens ability;
-To motivate children to be good in their school involvement;
-To encourage other parents to give value on education concern.
1X. Description:
The concern Municipal Link/Pantawid Worker are tasks to identify 4Ps dependent who
are good performer/Honor Student. There are Sixteen (16) qualified contestants from different
barangays. The search was rated according to the prescribed criteria furnished to us. School
card and other documents that shows students involvement in school and community activities
are also considered as plus factor. There are three (3) Board of Judges to rate the contestants.

X1. Recommendation:
This activity will promote encouragement to our students and it is one way of motivation
for them to be challenged to achieve good school performance. Thus, we hereby recommend

1X. Methodology:
The Supplementary Feeding Program is the provision of food in addition to
the regular meals to young children within 120 days to currently enrolled children
in a form of Hot Meal. Identification of beneficiaries will be done.DCWs,
Review, summary and submission is under the MSWD Office. Fund transfer will
be done by DSWD-09. Then weekly allocation will be released by the MSWDO
to concern Day Care Workers and the procurement of food items will be done by
the parents subject to DCWS supervision. After a week time, DCW will submit
liquidation reports and its pertinent documents. On the other hand, the parents
will be formed into sub-groups to handle the cooking and feeding activities..
X. Funding Requirements:
893 Children x P13.00x120 days= P 1,393,080.00
X1. Recommendation:
This program is found beneficial to our growing young children. It has a
multiple developmental contribution to the recipients. In addition, it will also
develop parents responsibility towards childrens nutrition, education and health
concerns. Relative to this, we hereby recommend for the approval of this
proposal.

III. Problem Presented:


Clients 45 year old husband named Romeo Sano undergone colostomy operation at
Zamboanga City Medical Center (ZCMC) last September, 2012. He was advised to go back
after one (1) year. Unfortunately, on September 2013 there is a problem regarding the peace
and order situation of the city. Thus, patient wasnt able to undergo laboratory test as it is
required. On September this year, patient again consulted his attending Physician and
undergone said test and it was found out that he is still physically weak and needs further
treatment for him to be patient is temporarily staying at Recodo, Zamboanga City since he is
an out-patient and they cannot afford to pay the board and lodging excluding other related
expenses.
IV. Findings:
They lived in barangay Villagracia, Siay, Zamboanga Sibugay, blessed with ten (10)
children of whom three (3) are already married while five (5) of them are all in school.. Client is
a mere house keeper doing household chores and maternal obligations while her husband is
engaged in lemon picking and her two (2) sons are employed in Zamboanga City as a janitor
and factory worker of which income is very limited to support for their primary needs. In
addition, the prevailing ailment of her husband is a great burden to them.
V. Recommendation:
Upon thorough assessment of the situation, client is found eligible to avail medical
assistance from Department of Social Welfare and Development Office (DSWD) through
SWAD Office-Ipil.

Client 14 year old son was a victim of stabbing incident last December 7, 2013 at about
6:30 in the evening. Unfortunately it caused internal injuries and patient immediately
undergone surgical intervention at Zamboanga City Medical Center on December 8. As such,
cost a lot of which its beyond their financial capability
IV. Findings
They reside at Purok Tambolig, Balucanan, Siay, Zamboanga Sibugay. Both client and
his wife engaged in Calamansi Picking as their main source of living, but its seasonal. Client
declared that their monthly income is just about One Thousand Five Hundred (P1,500.00) a
month wherein it cant even sufficiently support for their primary needs. Instead, they opted to
engaged backyard gardening with the support of his children during weekend. This will help a
lot for their daily subsistence. In some other ways, children will also engaged calamansi
picking during holidays and weekend just to augment for their school needs.
V. Recommendation:
In view hereof, client is found financially depressed. With the present situation of the
patient they cant really afford the involved amount for the hospital, medicines, surgeon and
other related expenses. With this, he is hereby highly recommended to avail any favor
(medical or financial) that could be of help to his prevailing proble

III. Problem Presented:


Client and her husband encountered vehicular accident last December 29, 2013 of
which both of them suffered fracture of their left leg. Their confinement period at Zamboanga
City Medical Center (ZCMC) Zamboanga City lasted for three (3) months of which client needs
crutches for her to walk since her leg is not yet fully recovered, she cannot even bend her
knee. Unfortunately, they cannot afford to buy for it since; they were still on the process of
paying their accounts from their relatives and private moneylenders.
IV. Findings
They reside in Barangay Balucanan, Siay, Zamboanga Sibugay, blessed with five (5)
siblings; two (2) daughter and three (3) sons. Four (4) of them are supposed to be in school
but due to the tragic incident happened only one (1) of her daughter is in school who is
enrolled in grade 111. Her two (2) sons stopped schooling and engaged in various farm labor
works to augment for their needs. Client husband is engaged in lowland farming however, he
could no longer perform his task due to his present situation.
V. Recommendation:
In view hereof, client is found financially depressed. With her present situation she
needs crutches for her total recovery. With this, she is hereby highly recommended to avail
financial assistance that could be of help to her prevailing problem.

II. Problem Presented:
Last December 31, 2013 clients niece named Elaine Gallarosa accidentally slides from
the table and fells down inside the big kawa with boiling water. Such incident caused soft
tissue injury 2nd degree burn. Thus, she was immediately admitted at Kabasalan Pathfinder
Hospital,Goodyear, Kabasalan, Zamboanga Sibugay. Her confinement period lasted for 7 days
from December 31, 2013-January 7, 2014. Client opted to engage money borrowings from
private money lenders for the patients medical needs. After discharged, she is still under
medication for her total recuperation which costs about Seven Hundred Fifty Pesos (P 750.00)
daily for a period of two (2) months. Hence seeking financial support from the office.
IV. Findings
They lived at Barangay Batu, Siay, Zamboanga Sibugay. Separated with her husband
five (5) years ago and gifted with one (1) son. Client is living together with her parents together
with her sister, the mother of the patient since their married life isnt so stable. Her elder sister,
the mother of the patient got separated with her 1st husband thus highly dependent from the
financial support of her parents as well as with her present live-in partner being engaged in
private driving of which monthly income is so minimal. Their source of living highly depends
from meat vending of which they are just earning more or less P500.00 a week. Said source
of income is so meager to suffice for their daily subsistence having eleven (11) dependents to
support to include the client. .
V. Recommendation:
After thorough assessment of clients situation, she is financially depressed. With this,
she is hereby highly recommended to avail financial assistance from Local Government Unit of
Siay through Municipal Social Welfare & Development Office in the amount of One Thousand
Pesos Only (P1,000.00).

Patient was diagnosed to have a Chronic Kidney Disease on January 2014 at private
hospital in Pagadian City. Since then, she is required to have her daily medications but recuperation is
still of no avail. With this, client decided to go to Cebu City for further work-up and assessment.
Thereat, she was confined at Vicente Sotto Medical Center Cebu City on March 23, 2015 until this
present. Unfortunately, her ailment become worst and the attending Physician advised her to undergo
for immediate operation. Luckily, client is a Philhealth card holder however; it was already utilized
during her 1st hospitalization. The family revealed that they are financially exhausted and cannot afford
to pay the expected surgical and or medical expenses. Hence, sought agencys assistance.
IV. Findings
Client is a bonafide resident of barangay Coloran, Siay, Zamboanga Sibugay. They belong to
the indigent sector in the area. The couple gifted with two (2) children of whom all of them had already
their own family and living below poverty threshold. Their means of living derives from upland farming,
particularly rubber planting and other upland crops of which income is very meager and it could hardly
suffice for their daily subsistence .In addition. Patient ailment adds financial and emotional burden in
the family. Her siblings and other relatives extended their financial support to her but they cant do it
regularly since they also have their respective families and their source of income is also unstable.
V. Recommendation:
In view hereof, Client is found financially depressed and suffered such mentioned disease
which needs immediate surgical intervention but they dont have financial access. With this, she is
highly recommended to avail of it from any Charitable Institutions and or National Political Leaders that
could be of help to her.

Client is seeking financial assistance in favor of his prevailing health problem. He had
undergone Kidney Transplant on July 29, 2012 at Western Mindanao Medical Center
Zamboanga City. Since then, patient is required to have his daily medication which costs more

capability. In addition, he needs also to submit a quarterly medical check-up. Family resources

IV. Findings:
Client is a bona fide resident of Barangay Mirangan, Siay,Zamboanga Sibugay, eldest brother
among his two (2) brother and four (4) sisters. They are living together with his live-in partner for almost
two (2) years and gifted with one (1) kid, the patient. His step mother is a plain housewife doing
household chores and maternal obligations while his father is a bus inspector with a meager income of
which it isnt stable and it could hardly suffice for their daily subsistence.
V. Recommendation:
In view hereof, client is found eligible to avail of any access for medical support. Thus,

immediate confinement at Zamboanga Sibugay Provincial Hospital (ZSPH) Ipil on November 20, 2014.

(ZCMC) for further work-up and assessment. Luckily, she made it through normal delivery but the baby

work-up and assessment of which she was confined thereat for nine (9) days from October 24November 02, 2014. Fortunately, the family is a Philhealth card holder. However, there is still
an excess of their total hospital bill amounted to Ten Thousand Two Hundred Eight (P10,
208.00) Pesos to exclude their account at Pathfinder Estate Hospital in the amount of Eleven
Thousand Pesos (P11,000.00).
IV. Findings:
They lived in barangay Batu, Siay, Zamboanga Sibugay, living together for 19 years and
luckily gifted with five (5) siblings of whom two (2) of them are in school. Client is engaged in
fishing activity while his wife is doing household chores and rearing of their kids. Said source of
income is very minimal and it could hardly cope-up for their daily needs having six (6)
dependents to feed to exclude the client.
V. Recommendation:
Upon thorough assessment of the situation, client is eligible to avail financial support

sari-sari- store in Poblacion Siay for them to live. With the nature of her work, she wasnt able

beyond their capacity to have such capital and or amount for the said purpose. Thus, sought
for financial assistance.

Client is a native of barangay Logpond, Siay, Zamboanga Sibugay, blessed with four (4)

are all in school except her eldest who stopped from studying due to financial difficulties.

of which he really experienced financial difficulties especially in his educational needs. With

His step-father is engaged in occasional carpentry works while his mother is serving as
a sales lady in one of the sari-sari store receiving a meager monthly salary. Said income is
very insufficient to answer for their entire primary needs having seven (7) dependents to feed.
V. Recommendation:
In view hereof, client is found eligible to avail educational assistance. Thus,

Client appeals agencys financial support in favor of her 19 year old son who met
vehicular accident on April , 2015 at around seven oclock in the evening while on their way
home. With that, he was immediately confined at Zamboanga del Sur Medical Center
(ZDSMC) Pagadian City. Thereat, patient underwent operation and or medical treatment under
Intensive Care Unit for three (3) days. His confinement period lasted for one (1) week from
August 01-07, 2013. Their total hospital bill amounted to One Hundred Forty Eight Thousand
Eight Hundred Ninety and Fifteen Centavo (P 148,890.15) Philhealth total deduction is Sixty
Five Thousand Eight Hundred Eighty and Forty Centavo (P65,880.40) Unfortunately, they cant
cope-up to pay the remaining balance in the amount of Eighty Three Thousand Nine and
Seventy Five Centavo (P83,009.75). With this, they borrowed big amount from their relatives
and private moneylenders with prescribed interest rate for them to be discharged. Hence, wish
to avail financial support from DSWD to pay their accounts.
IV. Findings.
Clients belong to the indigent sector of barangay Balucanan, Siay, Zamboanga
Sibugay, gifted with six (6) children four (4) sons and two (2) daughters of whom her eldest
daughter had already her own family while the three (3) of them are in school. Her husband is
engaged in various farm labor works while client is doing household chores and sometimes
engaged in lemon picking of which they are only earning a minimal income of which it is very
meager to suffice for their daily subsistence. In addition the recent incident that happened to
her son adds financial and emotional burden in the family.
V. Recommendation:
In view hereof, she is found economically unstable. Thus, she is hereby highly
recommended to avail financial support from Department of Social Welfare and Development
through SWADT Office-Ipil.

Clients two year old son, John Kenneth was found to have congenital Heart Ailment.,
three (3) weeks after birth. With this patient undergone several check up and 2 D echo last
March 10 and November 8, 2008.
Patient was hospitalized for about three (3) months at National Heart Center East
Avenue, Quezon City. After such, theyre advised to have quarterly medical monitoring and 2
D echo twice a year and continuous medication. Unfortunately, financial capability will not
warrant. Thus, wish to avail financial support from any concern agency.
IV. Findings.
After giving birth of their 5th child, John Kenneth in Bawang La Union, her husband
decided to go home to his hometown here in Siay particularly in barangay Mirangan to look for
a job. Unfortunately, her husband did not send any amount nor communicate with them. Thus,
client tapped the help of her relatives for them to survive since she has no income and she just
newly gave birth that time. For almost two (2) years her husband abandoned them thus, she
plays dual role in the family and recently she just arrived from La Union with her kids to look/
visit the situation of her husband. Unluckily, she found out that her husband has no stable work
thus his income is uncertain.
V. Recommendation:
In view hereof, client is found financially depressed. With this, she is hereby highly
recommended to avail financial support from the Local Government Unit of Siay through
Municipal Social Welfare And Development Office in the amount of One Thousand Pesos (P

problem that needs immediate surgical intervention. Hence, the Physician advised him to
undergo for operation and it was done last April 06, 2014 at the mentioned hospital. Patient
period of confinement lasted for more than two (2) weeks from April 06-17, 2014. Fortunately,
he is a Philhealth card holder however; the remaining amount from the Philhealth deductions is
One Hundred Twenty Eight Thousand Six Hundred Twenty Seven Only (128,627.00) with this,
the family indebted big amount from their relatives and private money lenders with high interest

Client and his wife encountered vehicular accident last December 29, 2013 of which
both of them suffered fracture of their left leg. However, client was affected most. Their
confinement at Zamboanga City Medical Center (ZCMC) Zamboanga City lasted for three (3)
months. Client full recovery is not attained since he is required to produce Forty Thousand
Pesos (P40,000.00) to buy e such for stainless to be installed in the fractured area.
However, its beyond their capacity to have such amount.
IV. Findings:
They reside in barangay Balucanan,Siay, Zamboanga Sibugay . He engaged in lowland
farming and various labor works which serves as their source of living. Theyre blessed with
five (5) children. Four (4) of them are supposed to be in school but due to the tragic incident
happened only one (1) child is now in school in the person of Juvy who is enrolled in Grade
111. Their son jomar and Aldrin stopped schooling and they serves as the household incharge
and care taker of the youngest child Lito.Her mother cannot walk without crutches and his left
leg is not yet fully recovered, she cannot even bend her knee.
V. Recommendation:
Per assessment made, client is found eligible to avail financial assistance that would
hopefully realize the procurement of stainless that may contribute for his full recovery. With
this, I am highly recommending him to avail of it through the office of congresswoman Dulce
Ann K. Hofer of 2nd District, Province of Zamboanga Sibugay.

Client 19 year old daughter is due to give birth but she experienced prolonged labor
which resulted her confinement at M. Simon Hospital Ipil, Zamboanga Sibugay of which
patient undergone caesarean operation last December 20-27,2012. . As such, they indebted
bigger amount for the medicines, surgeon, hospitalization and other related expenses. Their

They lived at barangay Coloran,Siay, Zamboanga Sibugay. Met and got married
sometimes in the year 1983 and blessed with four (4) siblings; two (2) are married while the
other still studying. Client expressed that her eldest daughter, the patient went to Manila to
look for a job. Fortunately, she was employed as a House Helper but she just only work for
only four (4) months since she already met her live-in partner and got pregnant. Thus, decided
to go back here in Siay together with the father of her newly born baby. Her husband is a mere
tenant cultivating more or less hectare of lowland farm of which income is in seasonal basis
while client is a plain housewife doing maternal obligations and sometimes helping in various
farm works. Income deriving from this source is so meager to suffice for their daily subsistence
having six (6) dependents to feed.
V. Recommendation:
Upon thorough assessment of the situation, client is in dire need of financial assistance.

cant even sufficiently provide their respective families. The prevailing ailment of clients
husband is a great burden for them.

recommended to avail free hospitalization and or Medical support from Medical Social
Services of Zamboanga City medical Center and or any concern office that could be of help to

the support of her nephew. She engaged banana cue vending, selling assorted rootcrops, raw
banana and other food related items. However, when she got to have such ailment, she
discontinue her livelihood activities.
Her husband is the sole bread earner in the family by engaging hectare coco
farming activities. The farm production is once in every quarter of which income is so minimal.
It cant cope up her medical needs. They occasionally received financial support from her two
(2) married children, however it cant sustain since they also have their respective families to
support with.
V. Recommendation:
Per assessment made client is found both emotionally and financially burdened due to
her prevailing ailment which requires continuous medication and health monitoring. With this,
she is hereby highly recommended to avail medical support from Novartis Healthcare
Philippines, Inc. (in any branch office).

IV. Findings.

activities due to his illness. They occasionally received financial support from their married
children but it cant sustain since they also have their respective families to support with.

highly recommended to avail financial assistance from Local Government Unit-Siay through

Clients 15 year old son named Ariel Villamora undergone appendicitis operation at
Zamboanga Sibugay Provincial Hospital . Patient confinement lasted for five (5) days from January 48, 2010. With the mentioned incident, the family indebted bigger amount from private moneylenders
for the medicines, surgeon and hospitalization. Unluckily, they can no longer pay their hospital bill
amounting to Twenty Three Thousand Five Hundred Ninety Pesos (P 23, 590.00). Familys resources
were already exhausted hence, sought agencys intervention.
IV. Findings.
The couple was gifted with eight (8) siblings; six (6) of them had already their respective
families while her younger daughter work as a house helper with a minimal salary at Zamboanga City
to augment familys income. Client engaged in nipa shingles making while her husband is cultivating
a small upland farm. Said income isnt sufficient to support their daily needs.
V. Recommendation:
Per assessment made, client is really in dire need of financially support. With this, she is
hereby highly recommended to avail financial assistance the Local Government Unit of Siay through
Municipal Social Welfare & Development Office in the amount of One Thousand Pesos Only ( P

They resides in barangay San Isidro, Siay, Zamboanga Sibugay. He engaged rubber tapping
as their main source of living. However, his income is just minimal, he will just be receiving one third
share from the gross sales. His average income per month is about Three Thousand Pesos Only
( P3,000.00). Such amount is not even enough for their primary needs. Besides, client and the baby
were also advised to under Hepa Test as precautionary measures. In addition, patient will also be
spending from time to time for medical monitoring purposes.
V. Recommendation:
Upon thorough assessment, client was found financially unstable. With this, he is highly
recommended to avail financial assistance from Zamboanga Sibugay Provincial Government through
Provl. Social Welfare and Development Office (PSWDO ) Ipil.

forehead right face and to open fracture (R) leg. With this, he was immediately confined at

In this connection, they engaged money borrowings from private money lenders with interest

IV. Findings.
Client resides in barangay Batu, Siay, ZSP. She engaged in lemon picking while his husband doing
various works but their income is very insufficient since their nature of work is irregular. In addition,
the prevailing ailment of his husband adds their financial problem of the family.

up their daily needs having six (6) dependents to feed to include her self.
V. Recommendation:
After assessment of clients situation, she is found financially burdened. With this,
undersigned highly recommend to avail financial assistance from Local Government Unit of Siay

III. Problem Presented:

Client appeal agencys financial support in favor of her 15 year old grand daughter, Mary Ann
Simoran who undergone appendicitis operation at Zamboanga Sibugay Provincial Hospital-Ipil.
Patients confinement lasted for six (6) days from March 11-16, 2010. With this, the family can no
longer afford to pay their hospital obligation amounting to Twenty Eight Thousand Six Hundred
fourteen ( P 28,614.00).
IV. Findings;

Client is a bona fide resident of barangay Monching, Siay, Zamboanga Sibugay. The couple gifted
with six (6) siblings; three (3) sons and three (3) daughters. Four (4) of them had already their own
family. Unfortunately, one of her married daughter got separated with her husband six (6) years ago.
Due to the difficulties in life, her daughter went to Manila to look for a job for her to provide the need
of her four (4) children of which she left with her parents. Unluckily, client didnt know her
whereabouts since she didnt communicate with them. Client two (2) daughters at present are
working as a helper with a minimal monthly salary to augment familys income. Her husband engaged
in shell gatherings which serve as their main source of living, while she performs maternal obligations
to their grandchildren and undertakes various household chores.
V. Recommendation:

In view hereof, client is found financially needy. With this, she is hereby recommended to avail
financial assistance from Local Government Unit of Siay through Municipal Social Welfare And
Development office in the amount of Five Hundred Pesos (P 500.00).

Clients daughter named Joy Felomino suffered mental ailment since September 2010. Patient
undergone several medications but recuperation is of no avail. It is their great desire to undergo
further psychiatric assessment and management. However, they cant cope up the expected medical
demand. Thus, sought agencys financial assistance.
Findings;
Client is a resident of barangay Coloy-Coloran, Siay, Zamboanga Sibugay, blessed with eight (8)
siblings; three (3) sons and five (5) daughter of whom three (3) of them had already their own family.
Patient started her ailment when they got misunderstanding with her live-in partner regarding their
son. Client is a plain housewife while her husband engaged in low land farming. Income deriving from
this source is so meager to support their daily needs. With the present incident that happened to her
daughter they engaged several money borrowings for her medications and hospitalizations.
V. Recommendation:
In view hereof, client is found financially needy. With this, she is hereby recommended to avail
financial assistance from Local Government Unit through Municipal Social Welfare and Development
Office in the amount of Five Hundred Pesos Only (P 500.00).

Clients 5 month old son was confined at Pathfinder Estate Hosptial, Kabasalan, Zamboanga
Sibugay due to polmonia and other complications. Their hospital bill amounted to Three thousand

this, she was forced to engage money borrowings but the amount they had secured cannot fully

and or various labor works activities just to earn daily and provide familys needs. However, his

Client is a fire victim. His house was made of light materials and razed to the ground by fire
last July 5, 2010. All of their personal belongings were totally burned. After the incident, they
temporarily stay with their concern neighbor. However, it is their great desire to re construct their
Nipa house but theyre financially constraint.
IV. Findings.

respective families except his eldest son. Their means source of living highly depend from upland
farming and various labor works. Said income is very insufficient to support basic needs of the family

Government Unit of Siay through Municipal Social Welfare & Development Office in the amount of

III. Problem Presented:


Client is seeking financial assistance in favor of her 63 year old husband who suffered liver
ailment and progressive mass on his large intestine with some other complications. with this he was
confined at Western Mindanao Medical Center (WMMC) Zamboanga City last February 08, 2010 until
February 17, 2010 of which they spent a lot, in fact their hospital bill amounted to Seventeen
Thousand Three Hundred Sixty Seven and Fifty Centavo (P 17,367.50) and some other related
expenses. Upon discharged, patients health condition is still unstable. He is temporarily staying with
her married daughter residing at Upper Calarian.
IV. Findings.
They resides at barangay Poblacion, Siay, Zamboanga Sibugay. Theyre blessed with eight (8)
children; of which six (6) of them are already married. Their source of living merely depends from
lowland farming of which its production is seasonal. However, farming activities was done through
hired laborer since family head (patient) suffered his ailment for almost a year.
On first week of February,2010 he went to Zamboanga City for check up and stays with his
married daughter. He was given prescription for his medication. However, it didnt work. Instead, he
was confined from January 8-17, 2010. He is supposed to undergo surgical intervention but failed to
submit for it due to financial constraints. At this time, theyre on the process of soliciting possible
financial support from his children, relatives and government agencies for them to raise an amount for
his operation.
V. Recommendation:

Per assessment made, patient ailment needs intensive treatment, but theyre financially
incapacitated. With this, she is hereby highly recommended to avail financial support from Local
Government Unit of Siay through Municipal Social Welfare and Development Office in the amount of
Five Hundred Pesos ( P 500..00).
