Scenario

During an audit, you found no statement of Quality Policy or

Quality Objectives. However, when conducting an interview
with the production workers, the auditee was able to clearly

stating the organization policy and objectives.

You found that all measurement equipments were calibrated,
with the evidence of calibration sticker and certificate. One
measurement equipment is found without sticker or certificate,
but the auditee explained that they have not received it yet

from the laboratory.

Auditor found that no evidence that the material to be
purchased is recorded in the Purchased Requisition Form
(PRF) as per procedure PHM-PRO-10, but every purchased
item is recorded in the stock book and Materials Received

Form (MRF).
Record kept in the record and filing room are scattered. You
ask the auditee to find one record, as the record number
provided by you. It takes the auditee 30 minutes to show it up

to you.
Uncontrolled documents were used by the management
representative as training tools to train the new staff. The
usage of it is unofficial. The MR explained that for staff
revision on the process work or official task, they can always
refer to the Controlled copy that being placed at the quality
corner.
