I, De Guia, Milton I._ of legal age of 37, Filipino, married and a resident of 4566
V. Francisco St, Sta Mesa, Manila, after first being duly sworn to in accordance with law,
do depose and say:
That the company car temporarily issued to me by Sony Philippines,Inc. with details
below , and more particularly described as follows:

That on the day of 7:30 am, while on my way to office, I was about to leave our house,
upon seeing the car parked on the street of our barangay, (overnight) I noticed that my
bumper fell on the ground as if something bump on it, but as I ask my neighbors and
street sweepers, they didnt noticed the said incident.
That I am executing this affidavit in order to attest to the truth of the foregoing facts and
for whatever purpose this may serve.
