I, BEVELYN JUNCO, married, of legal age, Filipino, and a
resident of Manbantao, Kapalong DAVAO DEL NORTE after being duly
sworn to in accordance with law, do hereby depose and state, that:
1. I am the President of Party and Join Option
Trader, INC;
2. The said company was incorporated on January
29, 2016 and has stopped its operation last April
2016 due to loss of income and/or no income;
3. The said company ceased to transact or has no
transaction if any, from the time it ceased to
operate;
4. I am attesting to the truth that our company has
no more transaction for the purpose of retiring the
said company from operation;
5. I am executing this Affidavit in good faith, for
whatever legal purpose it may serve and to attest
to the veracity of the foregoing;
